DECLARATION
of 17 October 2008
amending Decree No 671 / 2004 Coll., laying down the details of the organisation of the admission procedure for secondary school education, as amended
The Ministry of Education, Youth and Sports provides, pursuant to § 60 paragraphs 1, 6 and 8, § 60a paragraph 4 and § 64 of Act No. 561 / 2004 Coll., on pre-school, primary, secondary, higher vocational and other education (Education Act), as amended by Act No. 243 / 2008 Coll.:
Decree No 671 / 2004 Coll., laying down the details of the organisation of the admission procedure for secondary school education, as amended by Decree No 422 / 2006 Coll. and Decree No 46 / 2008 Coll., is amended as follows:
1. in Paragraph 1 (1) (f), the words "receiving secondary education with a graduate examination" shall be replaced by the words "obtaining a relevant secondary education degree."
2. in Paragraph 1 (2), including footnotes 6, 6a, 6b and 7:
"(2) The application also includes documents related to the criteria of the admission procedure laid down by the Director of the Central School6), which shall be determined by the School Director, such as the final assessment of the candidate from primary education6a), evidence of the results in professional competitions, of the publication activities, of the qualifications acquired 6b) or of the further training achieved in the School7).

Announcement of the first round of the admission procedure
The principal of the school shall declare the first round of the admission procedure in the fields of education with a talent test by 31 October at the latest, in the other fields of education by 31 January, by publication in a publicly accessible place at the school and in a way that allows remote access. ';
4. Paragraph 3, including the title and footnote 7b, reads:
Application procedure
(1) The entrance exam in the first round of the admission procedure, if the school director has decided on its conduct, shall take place on working days from 22 April to 7 May.
(2) Talent examination in the field of education with talent examination in the first round of the admission procedure takes place on working days from 2 to 15 January. Talent test in the Conservatory in the first round of the admission procedure shall take place on working days from 15 to 31 January.
(3) In the first round of the admission procedure to a non-daily form of education, to an extension study, to a shortened study for obtaining a secondary education with a certificate and to a shortened study for obtaining a secondary education with a graduate examination
(a) the entrance exam, if the principal of the school has decided on its conduct, on working days from 2 to 17 May; or
(b) a talent test at the dates referred to in paragraph 2.
(4) The days of the examination referred to in paragraphs 1 to 3 shall be determined by the school director.
(5) In the event of a joint recruitment procedure in several fields of education, the school director, together with the announcement of the admission procedure, shall publish this fact for the first round of the admission procedure as part of the notification provided for in Section 2.
(6) In the event of the announcement of further rounds of the admission procedure, the date by which applications for training may be submitted at the latest shall be announced.
(7) The invitation to take the entrance examination or the talent examination shall be sent by the principal of the school to the tenderer no later than 7 days before the examination.
(8) The invitation to receive the examination or the talent test shall include information on the requirements for these examinations, the expected number of candidates admitted and the criteria for the admission procedure, as determined by the school director. 7b)
7b) § 60 (3) (a) and (b) of Act No. 561 / 2004 Coll. ';
5. Paragraph 4 (1), including footnote 8, reads as follows:
"(1) The school director shall publish in a publicly accessible place at the school and in a way that allows remote access, in accordance with the legislation on the protection of personal data (8),
(a) the ranking of tenderers according to the outcome of the evaluation of the admission procedure;
(b) an overview of the tenderers received and not received;
(c) the criteria for the admission procedure.
8) For example Act No. 101 / 2000 Coll., on the protection of personal data and on the amendment of certain laws, as amended. '
6. In Paragraph 4, the following paragraph 2 is inserted after paragraph 1:
"(2) The data referred to in paragraph 1 shall be made public:
(a) within 3 working days of the last date determined by the school's principal for the entrance examination; or
(b) the entrance examination has not been established by the school director after evaluation of the admission procedure, not later than the last day of the deadline referred to in § 3 (1) or (3) and in other rounds without undue delay. "
Paragraphs 2 and 3 shall become paragraphs 3 and 4.
7. In Article 5 (1), the words "in schools established by the State, the county, the municipality or the association of municipalities' are deleted.
8. In Article 5, the following paragraph 3 is added:
"(3) The school director, which is not established by the State, the county, the municipality or the association of municipalities, shall proceed on appeal under the school's internal rules. '
9. the following Section 5a is inserted after Section 5, including the title and footnote 9:
Registration form
(1) The form of the entry note shall be accompanied by a grey copy of the leaves on each side of the form, a watermark with the motifs of the lip branches, a registration number containing a series of forms and at least a six-digit number of the form. The forms shall be drawn up on a sheet of 210 x 280 mm format with a tolerance of 3 mm.
(2) The model form for the entry note is set out in the Annex to this order. The place where the school's department identificator9) is indicated on the form by "IZO '. In case of repeated application of the entry note by the applicant at the third and other schools, the necessary information may be provided on the other side of the entry note.
(3) Registration of forms of registrations issued to individual tenderers is kept by the authority which issued them. The entry note is registered under the registration number for each applicant, which is issued in accordance with Section 60a of the Education Act. The Regional Office shall issue the entry note identifying the tenderer or legal representative of the minor on the basis of a verification of his identity, or send it to the tenderer or legal representative of the minor tenderer through the postal operator in its own hands.
(4) At the time of receipt of the registration form, the secondary school or conservatory shall verify that the information contained in the registration form corresponds to the information in the application form.
(5) In the event of loss or destruction of the registration, the authority which issued it shall issue a replacement registration, upon written request, without undue delay. The application for a replacement registration card shall include an affidavit from the applicant or the legal representative of the minor applicant that the original registration card has neither applied nor applied in the secondary school; The signature of the tenderer shall be part of the honorary declaration of the legal representative of the minor tenderer. The replacement entry note shall be marked "REFERENCE 'before the heading of the entry note. Paragraphs 1 to 4 shall apply mutatis mutandis to the replacement entry note.
9) § 144 (1) (a) of the Education Act. '
