The model of a civil servant's service card is set out in the Annex to this Order.

This Decision shall enter into force on 1 January 2018.

1. Description

a) The civil servant's service card is a white plastic card, size 85,60 x 53,98 mm with rounded corners with a radius of 2,88 to 3,48 mm.

(b) On the face of a civil servant's service card, the text "STAFF SERVICE TRADE" shall appear on the first to second lines. The third line contains the text "TITULE, NAME AND RECEIVATION: '. The fourth line contains the text" STATE EMPLOYMENT EVIDENCE NUMBER:'. The fifth line contains the text "DATE OF EXAMINATION:" and the text "VALIDITY TO: '. The sixth line contains the text" SERVICE AUTHORITY:'. In the left part there is digital processing of a photograph of a civil servant with dimensions of 26 x 34 mm, below which is the text "SERVICE CERTIFICATE NUMBER:." The top right is a big state emblem. The contact electronic chip may be located in the right part. Under each text on the third to seventh row, there is a white field used to write the relevant data. Data shall be entered in the white field under the third line in the format of the academic title given before the name, name or, where applicable, name, surname, academic title given after the name. The surname and designation of the Office shall be recorded in capital letters. The date of issue and the date of validity shall be indicated in the format dd.mm.yyyy. If the service card of a civil servant is not restricted, the text "without restriction 'shall be entered in the relevant white field. All texts, including written data, shall be in black in capricious characters. The background of the RS consists of a blue colour fluently passing into red at the bottom. The photograph of a civil servant is in color. The requirements for the technical processing of photographs comply with the technical requirements for photographs for the issue of the identity card.

(c) The reverse side of a civil servant's service card shall be reserved for the entry of other data, if otherwise provided for by other legislation.
