DECLARATION
of 17 December 2015
amending Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related acts, as amended
According to § 96 (a) and (e) of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related laws:
Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the Amendment of Certain Related Acts, as amended by Decree No. 230 / 2001 Coll., Decree No. 499 / 2001 Coll., Decree No. 566 / 2002 Coll., Decree No. 660 / 2004 Coll., Decree No. 539 / 2005 Coll., Decree No. 300 / 2006 Coll., Decree No. 352 / 2006 Coll., Decree No. 315 / 2010 Coll., Decree No. 417 / 2011 Coll., Decree No. 455 / 2012 Coll., Decree No. 445 / 2008 Coll., Decree No. 485 / 2009 Coll., Decree No. 315 / 2010 Coll., Decree No. 417 / 2011 Coll., Decree No. 455 / 2012 Coll., Decree No. 424 / 2013 and Decree No. 192 / 2014 Coll.
1. In Section 10, the words "surname 'shall be inserted after the word" sex'.
2. In Paragraph 10, the following paragraph 4 is inserted after paragraph 3:
"(4) In the minutes, the name of the registered person shall be filled in only if the surname currently used is different from that of the birth. ';
Paragraph 4 shall become paragraph 5.
3. In Annex No 1, in the part of the South Bohemian Region, in the column "Community with extended scope ', in point 3," Český Krumlov', in the column "Matrix Office 'in point 8," Kajov', in the column "List of municipalities, urban areas, urban districts and military exits of the administrative district of the Matrix Office ', point 2 is added after point (1)" Kajov'.
4. In Annex 1, in the part of the South Moravian Region, in the column "Community of extended application 'in point 2." Boskovice', in the column "Matrix Office 'in point 3 and in the column" List of municipalities, urban areas, urban districts and military exits of the administrative district of the Matrix Office' in point 1, the words "Books at Boskovice 'are replaced by the words" Books'.
5. In Annex No 1, in the part of the Karlovy Vary Region, in the column "Enlargement Community 'in point 3." Karlovy Vary', in the column "Matrix Office 'in point 2." Bochov', in the column "List of municipalities, urban areas, urban districts and military estates of the administrative district of the Matrix Office ', point 2 is inserted after point 1." Bochov'.
Point 2 is renumbered point 3.
6. In Annex No 1, in the part of the Karlovy Vary Region, in the column "Community of extended application ', in point 6," Island', in the column "Matrix Office ', in point 3," Island', in the column "List of municipalities, urban areas, urban districts and military exits of the administrative district of the Matrix Office ', point 1 is inserted. Points 1 to 6 shall become points 2 to 7.
7. In Annex 1, in the part of the Olomouc Region, in the column "Community with extended application ', in point 1." Boundary', in the column "Matrix Office ', in point 5." Puncture', in the column "List of municipalities, urban areas, urban districts and military estates of the administrative district of the Matrix Office ', the following point 1 is inserted:" Lubometer under the Strážnou'.
Point 1 is renumbered point 2.
8. In Annex 1, in the part of the Olomouc Region, in the column "Community with extended application ', in point 7." Olomouc', in the column "Matrix Office ', in point 18." Grand Escape', in the column "List of municipalities, urban areas, urban districts and military exits of the administrative district of the matrix office ', point 2 is inserted after point 1." Point 2 is renumbered point 3.

Points 13 to 19 shall become points 14 to 20.

Points 2 to 13 shall become points 1 to 12.
Efficacy
This Decision shall enter into force on 1 January 2016.
