on the model of the certificate of the Inspector of the State Institute for Drug Control pursuant to the Law on in vitro medical devices and diagnostic medical devices
The Ministry of Health provides, pursuant to Section 68 of Act No. 375 / 2022 Coll., on Medical Devices and Diagnostic Medical Devices in vitro, for the implementation of Section 51 (2) of the Act:
Subject matter
The model of the certificate of the inspector of the State Institute for Drug Control, who is responsible for carrying out the inspection under the law, is set out in the annex to this decree.
Efficacy
This decree shall become effective 15 days after its publication.

Model of licence of the inspector of the State Institute for Drug Control
1. Description
(a) On the face side of the licence, the name of the inspection body - "STATE CONSTITUTION FOR CONTROL," and in the upper left corner of its logo, the words "INSPECTORA GUIDELINE" appear on the second line, and the address of the control authority's registered office - "Šrobářova 48, 100 41 Praha 10." In the white rectangular field for the completion of the text, the text "Card Number 'is shown on the fourth line and on the fifth line the text" Title, name and surname' is given. Under the text on the fourth and fifth lines, the relevant figure shall be entered in the white field. There is a photograph of the inspector in the left side with dimensions 2,1 x 2,6 cm. The lower part of the licence contains the text on the sixth to eighth lines "This card entitles the inspector in accordance with Section 4 of Act No. 255 / 2012 Coll., on Control (Control Regulations), its holder to carry out control activities within the scope of the powers conferred on the State Institute for Drug Control." The Czech lion is shown in the form of watermark on the background of the face side of the licence.
(b) On the reverse side of the inspector's licence, the name of the inspection authority - "STATE CONSTITUTION FOR CONTROL IV" shall appear on the top of the white rectangular field on the first line. The following is the section indicating the specific scope of the mandate in accordance with the legislation governing the activities of the State Institute for Drug Control for which the licence is issued. The middle part of the white rectangular field contains the following text on the second to third lines: "The document of the inspector of the State Institute for Drug Control is a public document. Counterfeiting or falsifying this card is criminal '. On the fourth to fifth lines is the text" In the case of the finding of the licence please ask the finder about his surrender to the nearest office of the Police Department of the Czech Republic or to any workplace of the State Institute for Drug Control. "In the lower part there is the text on the sixth line" The card issued on "left," "Stamp and signature" on the right and a small state emblem of the Czech Republic in the middle. Under the text on the sixth line, the relevant figure shall be entered in the white field. The Czech lion is shown in the form of watermark against the background of the reverse side of the licence.
c) The inspector's card shall measure 9,5 x 7 cm.
