DECLARATION
of 1 November 2017
amending Decree No. 434 / 2004 Coll., on the details of the scope and content of the mandatory data to be entered in the National Register of Persons disagreeing with postmortem tissue and organ sampling
According to Article 18 (4) of Act No. 285 / 2002 Coll., on Donation, Collection and Transplantation of Tissue and Organ and on the Amendment of Certain Laws (Transplant Act), as amended by Act No. 296 / 2008 Coll. and Act No. 44 / 2013 Coll.:
Decree No. 434 / 2004 Coll., on the details of the scope and content of the mandatory data to the National Register of Persons not in agreement with postmortem tissue and organ collection, is amended as follows:
1. In the first sentence of Article 1 (2), the words "with limited legal capacity or persons deprived of legal capacity 'are replaced by the words" whose independence is restricted'.
2. In the first sentence of Paragraph 2 (1), the words "The Coordination Centre for the Department of Health Information Systems' are replaced by" The Institute of Health Information and Statistics of the Czech Republic '.
3. In the second sentence of Paragraph 2 (1), the word "Centre 'is replaced by" Institute of Health Information and Statistics of the Czech Republic'.
4. In Paragraph 2 (2), the words "The Coordinating Centre for the Department of Health Information Systems of Health Equipment 'are replaced by the words" The Institute of Health Information and Statistics of the Czech Republic, the Statistical Institute of Health Services Provider'.
5. In Paragraph 2 (3), the words "Coordinating Centre for Department Health Information Systems' are replaced by the words" Institute of Health Information and Statistics of the Czech Republic '.
6. In the Annex, point 7 is added to the list of institutions:
"The 7th uterus."
Efficacy
This Decree shall take effect on the day of its publication, with the exception of Article I (6), which shall take effect on 1 December 2017.
