DECLARATION
of 20 December 2019
on the model of the card of the staff member and of the civil servant responsible for electronic communications and postal services control
The Ministry of Industry and Trade provides, pursuant to Section 150 (2) of Act No. 127 / 2005 Coll., on Electronic Communications and on the amendment of certain related laws (Act on Electronic Communications), as amended by Act No. 110 / 2007 Coll., Act No. 153 / 2010 Coll., Act No. 468 / 2011 Coll., Act No. 214 / 2013 Coll. and Act No. 258 / 2014 Coll., as amended by Act No. 113 (8) of the Act on Electronic Communications, and under Section 41 (3) of Act No. 29 / 2000 Coll., on Postal Services and amending certain laws (Act on Postal Services), as amended by Act No. 95 / 2005 Coll., Act No. 221 / 2012 Coll., Act No. 258 / 2014 Coll., for Implementation of Section 37 (2) of the Postal Services Act:
The model of the civil servant's licence to carry out the control of electronic communications and postal services is set out in Annex 1 to this Decree. The card shall be placed on the reverse side of the civil servant's card under another legislation (1).
The model of the licence of the employee responsible for the control of electronic communications and postal services is set out in Annex 2 to this decree.
Repeal
Decree No. 209 / 2017 Coll., on the model of the card of the staff of the Czech Telecommunications Office responsible for the control of electronic communications and postal services, is hereby repealed.
Efficacy
This Regulation shall enter into force on 1 January 2020.

1. Description of the reverse side of the licence
On the reverse side of the civil servant card, the words "THE STATE EMPLOYMENT TRAINING OF ELECTRONIC COMMUNICATION AND POSTAL SERVICES 'shall appear on the first to second lines. On the third line are the words" The holder of a civil servant's card of the Czech Telecommunications Office. "The fourth line contains the words" Service card number', including the number in the box. The fifth and sixth lines contain the words "authorised person to carry out control of electronic communications and postal services'. The lower part on the seventh line on the left shows the place and date of issue by the words" In Prague on "and on the eighth and ninth lines on the right it says" The signature of the President of the Council of the Czech Telecommunications Office. "The logo of the Czech Telecommunications Office is placed in the middle of the licence and the words" Czech Telecommunications Office "in the colour version appear in the lower part.

Model of the licence of the staff member responsible for electronic communications and postal services control
1. Description of the licence
a) On the face side of the licence, digital processing of the colour photograph of the employee 26 x 34 mm is located on the upper left. The requirements for the technical execution of the photograph shall comply with the technical requirements for photographs for the issue of the identity card. In the central part of the licence, the name of the inspection authority shall appear on the first line as "THE CZECH TELECOMMUNICATION AUTHORITY '. The second line on the right shows the registration number of the licence. The third line shall bear the identification of the licence by the words" EMPLOYER TRADE'. The fourth line shows the name, surname and title of the employee. The fifth line in the middle shows the function of the employee. A large national emblem of the Czech Republic is located in the right part of the licence at the level of the fourth and fifth lines.
(b) On the reverse side of the licence, the words "EMPLOYMENT CERTIFICATE" shall appear on the first to third lines, the words "EMPLOYMENT OF THE EMPLOYMENT OF THE EMPLOYMENT OF THE EMPLOYMENT," and on the fifth line the word "Ev. No." including the number in the box. On the sixth line, the words "authorised person for the control of electronic communications and postal services' shall appear. The lower part on the seventh line on the left shows the place and date of issue by the words" In Prague on "and on the eighth and ninth lines on the right it says" The signature of the President of the Council of the Czech Telecommunications Office. "The logo of the Czech Telecommunications Office is placed in the middle of the licence and the words" Czech Telecommunications Office "in the colour version appear in the lower part.
c) The card is printed on a white plastic adhesive film placed on a plastic card with dimensions of 85,60 x 53,98 mm with rounded corners with a radius of 2,88 to 3,48 mm.
