laying down the model of the electronic form for the application for a decision of the dispute on the obligation to carry out the transaction and the technical details of its use
Pursuant to Section 150 (5) of Act No. 127 / 2005 Coll., on Electronic Communications and on the amendment of certain related laws (the Act on Electronic Communications), as amended, (the Act), for the implementation of Section 129 (2) of the Act, as amended by Act No. 153 / 2010 Coll.:
The model of the electronic form (hereinafter referred to as the "form ') of the application for a decision of a dispute pursuant to § 129 (1) or § 129a (1) of the Act relating to the obligation of a participant or user to carry out cash is set out in the Annex to this decree.
(1) The Office's form will be made available via the Public Administration Information System in the form of an online HTML (HyperText Markup Language) form with a PDF output (Portable Document Format). The form can also be sent from the information system of the person performing the communication activity via the application program interface (API) after authentication based on the API key issued by the Office.
(2) The form shall indicate the items the completion of which is mandatory.
(3) The size of the data file containing the form, including all attached annexes, must be such as to enable an electronic submission to be made, the maximum aggregate size of which is 20 MB in case of submission to an e-mail address, 100 MB in case of another electronic submission.
This Decree shall take effect on 1 January 2011.
