GOVERNMENT REGULATION
of 10 July 2002
laying down the model of the service of a member of the Security Information Service
The Government orders the implementation of Act No. 154 / 1994 Coll., on the Security Information Service, as amended by Act No. 160 / 1995 Coll. and Act No. 155 / 2000 Coll.:
§ 1
The model of the service card of a member of the Security Information Service (hereinafter referred to as "the Service Card ') and its description are set out in the Annex to this Regulation.
§ 2
If a member temporarily fails to fulfil the tasks of the Security Information Service, the Security Information Service shall indicate the service card in the lower half of the front page with the red sign "NON-COMPETENT SERVICE TASKS."
§ 3
Government measure No OP 06 / 95 of 25 January 1995 declaring the representation of a member of the Security Information Service, including the badge which forms an integral part of it, and its description, published in the Sb.
§ 4
This Regulation shall enter into force on the day of its publication.

Front of the service card

Back of the service card

The service pass is 54 mm high and 86 mm wide. It's printed on a white plastic card.
The front is printed in green and blue, leaving 4 white rectangular fields for the inscription "Security Information Service," a photograph of the member of the Security Information Service, the registration number of the service card and its date of issue. In the lower right-hand corner of the front is indicated the title of the Director of the Security Information Service and its own signature. A square holographic sticker is affixed in the middle of the front of the service card. The entire front is covered with a transparent holographic lamination film; at its lower edge, there is a recurring wavy sign "Security Information Service."
The back of the service card shall contain a typescript of the name and surname of the member of the Security Information Service.
