GOVERNMENT REGULATION
of 3 October 2012
amending Government Regulation No 109 / 2008 Coll., on the conditions for the provision of special prices for publicly available telephone services
The Government mandates pursuant to Sections 38 (8) and 43 (5) of Act No. 127 / 2005 Coll., on Electronic Communications and on the amendment of certain related laws (the Act on Electronic Communications), as amended by Act No. 261 / 2007 Coll., hereinafter referred to as "the Act ':
Government Decree No 109 / 2008 Coll., on the conditions for the provision of special prices for publicly available telephone services, is amended as follows:

A person with special social needs for the demonstration of disability referred to in Paragraph 43 (4) of the Act shall submit the following documents for inspection:
(a) in the case referred to in § 43 (4) (a) of the Act
1. the ZTP licence according to Act No. 100 / 1988 Coll., on Social Security, as effective until 31 December 2011, and the doctor's certificate of practical or complete deafness; the ZTP licence can be demonstrated by the disability only until its expiry date, but no later than 31 December 2015,
2. a decision to grant a level II exceptional benefit because of complete or practical deafness; or
3. a ZTP card with a symbol of the designation of a person with complete or practical deafness pursuant to § 34 (4) of Act No. 329 / 2011 Coll., on the provision of benefits to disabled persons and on the amendment of related laws,
(b) in the case referred to in § 43 (4) (b) of the Act
1. the ZTP / P licence under Act No. 100 / 1988 Coll., as effective until 31 December 2011; the ZTP / P licence can be demonstrated by the disability only until its expiry but no later than 31 December 2015,
2. a decision to grant exceptional advantages in grade III; or
3. the ZTP / P licence pursuant to § 34 paragraph 5 of Act No. 329 / 2011 Coll.,
(c) in the case referred to in Article 43 (4) (c) of the Act, the notification or decision to grant a care allowance at the level of dependency II, III or IV; if the minor is a minor, the participant in whose personal care the minor is, shall provide proof that he is the parent of that person or the decision of the competent authority referred to in Article 7 (10) (a), (b), (d) or (e) of the State Social Support Act;
(d) in the case referred to in Article 43 (4) (d) of the Act, the documents referred to in (a) or (b) issued to the minor who is raised or personally cared for by the participant; the participant in whose personal care the minor is present proof that he is the parent of that person or the decision of the competent authority pursuant to Article 7 (10) (a), (b), (d) or (e) of the State Social Support Act. ';
2. footnotes 1 to 4 are deleted.
Efficacy
This Regulation shall enter into force on the 15th day following its publication.
