laying down the models for the census forms for the 2001 census, houses and flats and the model for the census commissioner and the census supervisor
The Czech Statistical Office provides pursuant to § 22 for the implementation of § 7 (3) and § 9 (3) of Act No. 158 / 1999 Coll., on the census of people, houses and flats in 2001:
(1) The model of the census form The census sheet of persons is given in Annex 1.
(2) The model of the census form is given in Annex 2.
(3) The model of the census form is set out in Annex 3.
The model of the census commissioner's card is set out in Annex 4.
The model of the census supervisor licence is set out in Annex 5.
This decree shall take effect on the day of its publication.
