DECLARATION
of 24 October 2016
on admission to secondary education
The Ministry of Education, Youth and Sports provides pursuant to § 28 (6), § 60g (4) and § 64 of Act No. 561 / 2004 Coll., on Pre-school, Primary, Secondary, Higher Vocational and Other Education (Education Act), as amended by Act No. 383 / 2005 Coll., Act No. 49 / 2009 Coll. and Act No. 178 / 2016 Coll.:
ORGANISATION OF THE ACCEPTANCE PROCEDURE TO THE FIRST YEAR OF EDUCATION IN THE CENTRAL SCHOOL
Forms of application for education
(1) These documents, or certified copies thereof, are part of the training application (hereinafter referred to as the application), unless otherwise specified,
(a) a certificate from the last two years in which the applicant has completed or has completed compulsory schooling or from the corresponding primary school years, even after completion of compulsory schooling;
(b) a certificate from the last two years in which the applicant has completed or terminated basic education if the tenderer has obtained or received basic education before completing compulsory education;
(c) a certificate from the sixth and seventh grade of primary school, or from the fourth and fifth grade of primary school, if the applicant applies to the first grade of the six-year high school, or eight-year high school,
(d) a certificate from the last two years in which the applicant has performed or completed compulsory schooling if the applicant has applied for the first year of the eight-year or six-year conservatory education programme;
(e) documents relating to the acquisition of secondary education with a certificate, if the applicant applies to the extension studies;
(f) proof of obtaining the appropriate degree of secondary education, if the applicant applies for a short course of secondary education with a certificate or for a short course of secondary education with a graduate examination;
(g) a medical assessment on medical fitness for education1) if, under the Government Regulation on the system of education in basic, secondary and higher vocational education, the admission of a candidate to education is a prerequisite for fulfilling the conditions of medical fitness of the applicant for the relevant field of education;
(h) the recommendation of the educational advisory establishment containing support measures for the necessary adjustments to the admission procedure, if it is for candidates with special educational needs;
(i) proof of completion of compulsory schooling, if the applicant has completed or completed compulsory schooling in a foreign school, issued by a foreign school; or
(j) a certificate of recognition of the equivalence of a foreign certificate issued by a foreign school or a decision to recognise the validity of a foreign certificate in the event that an appropriate degree of education is required for admission.
(2) The application may not include the document or an officially certified copy referred to in points (a) to (d) of paragraph 1 if the classification from the relevant years on the application is confirmed for those years by the school in which the applicant has completed or completed compulsory schooling or, where appropriate, completed or terminated basic education. The classification in the application for secondary education verified by the school shall be confirmed by the stamp and signature of the principal of the school or his authorised staff.
(3) A candidate who has not been issued the document referred to in points (a) to (f), (i) and (j) of paragraph 1 before filing the application, shall submit the document to the secondary school at the latest on the day on which he becomes a pupil.
(4) In addition, the application shall include documents proving that the criteria laid down by the school director have been met.
(5) The applicant who submits two applications for the first round of the admission procedure shall indicate on both applications from the school, the fields of education and, where appropriate, the focus of the school curriculum in the same order.
Invitation to the admission test
(1) The invitation to take an entrance examination or a talent test shall include in particular information on the expected number of candidates admitted and the criteria for the admission procedure laid down by the school director, the place and time of the entrance examination, the content of the entrance examination, the field of training to which the admission examination will be conducted, the form of the entrance examination and the list of authorised aids for its performance.
(2) The invitation to take a single examination or a talent test in the first round of the admission procedure shall be sent by the school director to the tenderer no later than 14 days before the date of the examination.
(3) The invitation to take the entrance exam or the talent exam shall be sent to the tenderer no later than 7 days before the examination.
School entrance exam
(1) In particular, the school director shall determine the date and place of the school entrance examination, its form, test subjects and the content and scope of the teacher, which shall not exceed the educational content of the Master Education Programme for primary education in the admission procedure for the four-year secondary education fields and the relative educational content of the Master Education Programme for primary education in the six-year and eight-year secondary education fields.
(2) The dates of the school entrance exam shall be determined by the school director so as not to overlap with the dates of the uniform examination.
Form, content and scope of uniform test
(1) Written tests of the single test contain closed test tasks with the offer of a response, open test tasks without the offer of a response, or wide open task with assessment of the solution process.
(2) The uniform test tests are carried out in the range
(a) the teaching provided for the first stage of primary school in the eight-year fields of education;
(b) teaching up to the seventh year of primary school, given the relative educational content of the Framework Training Programme for Basic Education in the six-year fields of education;
(c) teaching the entire Framework Education Programme for primary education in other fields of education.
Transmission of data from Centre applications
(1) The School shall transmit to the Candidate Register, through the Centre's information system, data from the application for a single examination. The Centre shall, through the Centre's information system, make available to schools by the end of February information on the format and manner in which data are entered in the candidate register.
(2) The School shall transmit the following information to the Centre in the candidate register:
(a) the name and, where appropriate, the names and surnames, the birth number or, if not allocated, the date of birth;
(b) the identification of the school and the field of education to which the applicant applies;
(c) the order of the dates of the uniform examination referred to in the application to be held by the tenderer; and
(d) data on the adaptation of the conditions for admission to the uniform test, which correspond to the degree of support measures (2) of the recommendation of the educational institution and other elements. The request for processing the award in Braille shall be transmitted by the Director of the School to the Centre without delay after the receipt of the application of the applicant from which such a recommendation from the educational institution results, no later than 2 working days after the deadline under Section 60a (5) of the Education Act.
(3) The Centre shall process the test documentation on the basis of the application data.
(4) For the replacement period, the school shall transmit the data referred to in paragraph 2 to the Centre no later than 2 working days after the receipt of an apology which complies with the requirements laid down in Section 60c (3) of the Education Act.
Test documentation of uniform test
(1) The Centre shall deliver the safety-deposit test documentation to schools for the first and second dates of the single test, not later than the last working day preceding the date of the single test.
(2) The test documentation consists of test notebooks, record sheets for writing the test solution and other documents and files relating to the single test for individual tenderers.
(3) Other documents relating to the single test are:
(a) the list of tenderers,
(b) a report on the conduct of the uniform test in the classroom;
(c) guidelines for ensuring a uniform test in the classroom; and
(d) other auxiliary materials.
(4) The head of the school or his authorised staff shall take over the safety deposit box from the person authorised by the Centre and check its integrity. Both take-overs will be confirmed by a transfer record.
(5) For the replacement term of the uniform examination, the Centre shall make available to schools the test documentation for the award of the uniform examination for individual applicants and other documents through the Centre's information system no later than 2 hours before the uniform examination is carried out within a replacement period set by the Ministry.
Duration and duration of the uniform examination at school
(1) The teaching teacher shall ensure that the uniform examination is carried out in the classroom. The test documentation shall be taken over immediately before the start of the uniform test by the awarding teacher. Furthermore, the awarding teacher shall proceed in accordance with the instructions for ensuring the conduct of the uniform examination in the classroom.
(2) The Czech language and literature test takes 60 minutes. The math test takes 70 minutes.
(3) During the single test, tenderers may use only authorised aids set by the Centre.
(4) The report on the conduct of the uniform examination in the classroom shall state any circumstances which may affect its course and evaluation, including any objections to tenderers applied immediately after the completion of the entrance examination.
(5) Record sheets and other documents are stored in the school in the manner laid down by the Charter Act and the File Service. The time limit shall be at least 5 years after the end of the admission procedure.
Protection of information to the public
(1) Information not publicly accessible is part of the award of the uniform examination, which is marked by the text "Public Inaccessible Information pursuant to Sections 60b (3) and 80b of the Education Act."
(2) The Centre will identify as information publicly inaccessible the award of tests from the Czech language and literature and from mathematics based on the level of the test role at the time of the decision of the Centre on its use for the award of the uniform examination.
(3) The information referred to in paragraph 2 shall cease to be a publicly inaccessible moment when tests from the Czech language and literature and mathematics are transmitted by the contracting teacher to the candidates in the classroom.
(4) A dossier containing information on the measures taken to ensure the protection of information which is not publicly accessible and other facts the disclosure of which would render the protection of information which is not publicly accessible pursuant to paragraph 2 ineffective may be declared as publicly accessible.
Security and organisational incident and resolution
(1) A security incident means an event where a person who is not entitled to do so becomes familiar with the information identified as publicly accessible. If a safety incident occurs during a single examination or at the time the safety deposit with the test documentation is stored at the school, or if there is knowledge of the fact that the safety incident occurred at the school during that period, the school director shall immediately notify the Centre.
(2) Where a security incident threatens the proper conduct of a single test, the tenderers concerned by the security incident shall carry out a single test within the time limit laid down in Article 60c (1) and (3) of the Education Act.
(3) An organisational incident shall mean an event which prevents the proper conduct of a single test or part thereof at school and is not a security incident within the meaning of paragraph 1. If an organisational incident occurs during a single examination, or part thereof, at a school, the school director shall immediately notify the Centre.
(4) Where an organisational incident endangers the proper conduct of a single examination, or part thereof, at a school, the Centre shall, after consultation with the school director, determine the further procedure by which the proper conduct of the single examination is ensured. If necessary, after consultation with the Centre, the Director of the School shall ensure that a uniform examination is held within the time limit laid down in § 60c (1) and (3) of the Education Act.
Transmission of data from the Centre record sheets
The school will transfer the record sheets to the electronic form and send them to the Centre through the Centre's information system without undue delay.
Doing a uniform examination in the field of education Gymnasium with sport preparation
In the first round of the admission procedure, a candidate applying for an education course of the Gymnasium with sport training shall take a uniform examination at this school. The result of the uniform examination will be made available to both the Centre and the schools, whose fields of secondary education with a non-talent examination will be submitted by the applicant in the first round of the admission procedure under Section 62 (7) of the Education Act.
Further wheel admission procedure
The applicant may apply to several schools, fields of education and, where appropriate, to the curriculum by the deadline set by the school. In the application, the applicant lists only 1 school, a field of education or a different focus of the school curriculum.
Adjustment of conditions for applicants with special educational needs
(1) The Director of the School shall adjust the conditions in the recruitment procedure for applicants with special educational needs according to the recommendations of the educational institution.
(2) The Director of the School shall, in cooperation with the Centre, adjust the test documentation of the uniform examination in accordance with the recommendations of the school advisory establishment.
(3) For those applicants for whom the recommendation of a school guidance establishment indicates a requirement to enlarge the font or for whom it is recommended to adapt the conditions of the computer use test, the Centre shall provide the school director with an open format test book immediately prior to the examination. The principal of the school is responsible for editing the test book.
(4) The Centre shall prepare a test book in Braille for applicants with visual impairment for whom the recommendation of the educational advisory establishment indicates a requirement for entry in Braille.
Modification of evaluation of uniform test results of foreigners
The Director of the School for the Evaluation of the Results of the Single Exam of Foreigners who do not pass the examination of Czech Language and Literature in accordance with § 20 (4) of the School Act creates, in cooperation with the Centre, the ranking of candidates on the basis of a reduced evaluation of all applicants accepted in the admission procedure in the field of education or a focus according to the school curriculum. Reduced evaluation does not contain the result of the Czech language and literature test. The order of the candidates in the reduced evaluation shall be used for their inclusion in the final order of the candidates established under Section 60d (3) of the Education Act.
Publication of the results of the admission procedure
(1) The school director shall publish in a publicly accessible place at the school and in a way that allows remote access
(a) the ranking of tenderers according to the outcome of the evaluation in the recruitment procedure; and
(b) the criteria for the admission procedure.
(2) The order of tenderers shall be published under the registration number. The registration number shall be assigned to the candidate at the school at the latest on the day of the entrance examination or the talent examination before the start of the examination. In the event that the entrance examination is not held or is not required to take place in accordance with the criteria of the admission procedure, the applicant shall be informed in writing by the principal of the school at the same time as the information on the absence of the entrance examination.
Appeals
(1) The number of vacancies for appeal procedures shall be determined by the school director.
(2) The school director shall forward to the Regional Office, together with the appeal and administrative file, the criteria for the admission procedure under which he followed the recruitment procedure or other documents relevant to the decision on appeal.
Registration form
(1) The form of the entry note shall be accompanied by a grey copy of the leaves on each side of the form, a watermark with the motifs of the lip branches, a registration number containing a series of forms and at least a six-digit number of the form. The forms shall be drawn up on a sheet of 210 x 297 mm format with a tolerance of 3 mm.
(2) The model form for the entry note is set out in the Annex to this order. The place of listing of the school department identifier shall be indicated on the form by the abbreviation "IZO '.
(3) Registration of forms of registrations issued to individual tenderers is kept by the authority which issued them. The entry note is registered under the registration number for each applicant, which is issued in accordance with § 60g of the Education Act. The School or Regional Office shall issue a registration card identifying the applicant or the legal representative of the tenderer on the basis of an identity check, or send it to the applicant or the legal representative of the minor at the request of the postal operator.
(4) At the time of receipt of the registration form, the secondary school or conservatory shall verify that the information contained in the registration form corresponds to the information in the application form.
(5) In the event of loss or destruction of the registration, the authority which issued it shall issue a replacement registration, upon written request, without undue delay. The application for a replacement registration card shall include an affidavit from the applicant or the legal representative of the minor applicant that the original registration card has neither applied nor applied in the secondary school; The signature of the tenderer shall be part of the honorary declaration of the legal representative of the minor tenderer. The replacement entry note shall be marked "REFERENCE 'before the heading of the entry note. Paragraphs 1 to 4 shall apply mutatis mutandis to the replacement entry note.
(6) In the event of a coincidence in the ways in which the entry note may be withdrawn under Section 60g (7) of the Education Act, information on the entry of the candidate to another school on the entry note may be provided on the other side of the form.
(7) The entry note shall be valid for the school year, which shall be marked above the name and surname of the applicant. The year for which the registration card is issued shall be indicated in the register of the primary school or regional office.
ORGANISATION OF THE ACCESSION PROCEDURE TO THE HIGH ANNUAL
The application shall include, in particular, the documents or certified copies thereof referred to in Article 1 (1) (g), (h) and (j) and in accordance with Article 1 (5), as well as the documents of the applicant for prior training for the determination of the form, content and criteria of the admission procedure and of the year to which the tenderer will be classified.
COMMON, TRANSITIONAL AND REPEAL PROVISIONS
The provisions of this Order, with the exception of those concerning the admission examination, shall apply mutatis mutandis to admission to education in the Conservatory.
The forms of the entry note, produced before the date of entry into force of this Order under the current legislation, may be used until their stocks are exhausted, provided that they are supplemented by the newly determined data in the form set out in the Annex to this Order, but no later than 31 August 2017.
The following shall be deleted:

EFFECTIVE
This decree shall take effect on the day of its publication.
