• brain and parasasal sinuses, unless photon therapy can ensure the planned dose for target volumes and compliance with dose limits for surrounding tissues and organs,
• pelvic neoplasms, unless photon therapy can ensure the planned dose for target volumes and compliance with dose limits for surrounding tissues and organs,
• paraspinally stored sarcomas and other tumors, unless photon therapy can ensure the planned dose for target volumes and compliance with dose limits on surrounding tissues and organs,
• in selected cases of interthoracic lymphoma, unless photon therapy can ensure the planned dose for target volumes and compliance with dose limits for surrounding tissues and organs,

Proton radiotherapy in adult patients

• the impossibility of maintaining dose limits on healthy tissues even if modern techniques of photon radiotherapy (IMRT, stereotactic and robotic radiotherapy, radiosurgery, tomotherapy) are used, unless photon therapy is possible to ensure the planned dose for target volumes and compliance with dose limits on surrounding tissues and organs. Increasing the personnel costs of performance carriers
The calculation of the point value of health performance does not include the item of the increase in the personal costs of the performance carriers (hereinafter referred to as the "increase ').
The increase shall be paid by the health insurance undertaking on the basis of the power-holders' time and the value of the wage index of the power-holders for each power declared.
The increase shall be calculated as the sum of the product of the minute rate of the power holders in the given power, the value corresponding to the index of the given bearer, the time of the given power-holder and the value of the increase of 0,1. The increase shall be rounded to the whole points and added to the direct cost of performance.
