DECLARATION
of 14 November 2024
on the service card of a National Security Office employee
The National Security Authority shall establish, pursuant to Section 138 (5) of Act No. 412 / 2005 Coll., on the Protection of classified information and on security competence, as amended by Act No. 267 / 2024 Coll.:
The model of the service card of a National Security Office employee (hereinafter referred to as "the service card ') is set out in the Annex to this Order.
Efficacy
This Decree shall take effect on 1 January 2025.

Model of service cards
1. Description
a) The service card is a blue plastic card, dimensions 85,60 x 53,98 mm with rounded corners with a radius of 2,88 to 3,48 mm.
(b) On the face side of the service card, the text "STAFF STAFF OF THE NATIONAL SAFETY AUTHORITY" shall appear on the first to second lines. The third line contains the text "RECEIVATION '. The fourth line contains the text" NAME'. The fifth line contains the text "TITULL." The sixth line contains the text "DATE OF EXAMINATION '. The seventh line contains the text" VALIDITY DO'. On the eighth line, the text "SERVICE TRADE INDUSTRY NUMBER 'appears. In the left part there is a space for digital photo processing of the staff member with dimensions of 26 x 34 mm, below which the text" SERVICE TRADE NUMBER "is given on the ninth line. The top right is a big state emblem. On the bottom right is the tenth line of the text" Director of the National Security Office. "Under each text on the third to ninth lines and above the tenth line there is a field used to write the relevant data. All texts, including written data, shall be in black in capricious characters. The background of the RS consists of blue colour fluently transforming at the bottom into white, red and yellow marking. The employee's photo is in color. The requirements for the technical processing of photographs comply with the technical requirements for photographs for the issue of the identity card.
c) On the reverse side of the service card there is a colour logo on the top right of the National Security Office. On the reverse side of the service card is inserted the depiction of a double-tail lion on a grey background. On the reverse side of the service card, the text "The National Security Office employee's service card is issued in accordance with Section 138 (5) of Act No. 412 / 2005 Coll., on the protection of classified information and on security competence and certifies that the holder is an employee of the National Security Office. Please hand over your service card to the nearest Czech Police Department or the National Security Office."
