DECLARATION
of 18 May 2004
laying down a model form pursuant to Section 5 (5) of Act No. 61 / 1996 Coll., on certain measures against the legalisation of proceeds from crime and amending and supplementing related laws
According to Article 20 (2) of Act No. 61 / 1996 Coll., on certain measures against the legalisation of proceeds from crime and amending and supplementing related laws, as amended by Act No. 159 / 2000 Coll. and Act No. 284 / 2004 Coll., hereinafter referred to as "the Act":
(1) The notification to the customs office of import and export provided for in Article 5 (1) and (2) of the Act shall be made by natural and legal persons on a form, the specimen of which is given in the Czech, English and Russian languages in the Annex to this Order.
(2) The notification form referred to in paragraph 1 is available at any customs office and in electronic form on the Ministry of Finance website / fight against money laundering.
(3) The notification form must be completed legibly in one of the official languages of the European Communities or in the Russian language. In addition to the signature, it may be completed by machine or printing; when filling in hands, it shall always be completed in capital letters.
(4) The notification form shall complete all data relating to a specific import or export case. The disclosure of inaccurate, unreadable or incomplete information shall be considered as non-compliance with the notification requirement.
Decree No 224 / 2000 Coll., laying down a model form for the notification of imports and exports of banknotes and coins, travel cheques or cash exchange notes for cash to the customs office, is hereby repealed.
This Decree shall take effect on 1 September 2004.
