DECLARATION
of 5 November 2024
on the model of service card staff of the Czech Trade Inspection Agency
The Ministry of Industry and Trade provides pursuant to Section 4 of Act No. 64 / 1986 Coll., on Czech Trade Inspection, as amended by Act No. 64 / 2014 Coll.:
The model of the service card of the Czech Trade Inspection Staff responsible for carrying out its inspection tasks is set out in the annex to this Order.
Repeal
Decree No. 65 / 2014 Coll., on the model of service card of a Czech Trade Inspection employee, is hereby repealed.
Efficacy
This Decree shall take effect on 1 January 2025.

Annex
Model of staff's service
1. Description
a) The employee's service card is a double-sided plastic card with 86 x 54 mm corners rounded. The printing of both face and reverse pages is made on white background. The service pass is secured with a cover sheet.
(b) On the front of the staff member's service card, the name of the inspection authority - "CZECH COMMERCIAL INSPECTION" shall appear on the first line in the blue field in white. The second line contains the text "SERVICE TRADE '. The third line shall contain the text" Number of service cards:'. The fourth line contains the text "Title: '. The fifth line contains the text" Name:'. On the sixth line is the text "Surname: '. The seventh line contains the text" Function:'. On the eighth line, the text "Inspectorate: 'is given. In the left part there is digital processing of the colour photo of the employee with dimensions 26 x 34 mm. The lower part contains the text" The certificate entitles the holder to carry out the inspection activities under the laws governing the activities of the Czech Trade Inspection Office. "
c) On the reverse of the staff member's service card, the upper part is the text "The Czech Trade Inspection Service Card is a public document. Counterfeiting or amending an authentic instrument is criminal. Abuse of the Czech Trade Inspection Service Card is criminal. The ticket finder is obliged to submit it to the nearest office of the Police of the Czech Republic or to any of the inspectors of the Czech Trade Inspection Office." In the lower left corner there is a logo containing the abbreviation "CHOI 'and the text" CZECH COMMERCIAL INSPECTION'. The text "Issued on." The text "Signature of the service authority 'is given in the lower right-hand corner.
