on the publication of documents certifying acquired rights of formal qualifications or formal qualifications for the relevant field of specialisation of doctors, dentists and pharmacists awarded in the territory of the Member States of the European Union
The Ministry of Health pursuant to § 26 (2) and (3) of Act No. 95 / 2004 Coll., on the conditions for obtaining and recognising professional competence and specialised competence for the pursuit of the medical profession of physician, dental practitioner and pharmacist, declares:
- in Part One, documents certifying acquired rights to formal qualifications (professional qualifications) for the profession of doctor or formal qualifications for the relevant field of specialisation of doctor (professional),
- in Part Two, documents certifying acquired rights to formal qualifications (professional qualifications) for the independent exercise of the profession of dental practitioner or formal qualifications for the relevant field of specialisation of dental practitioner (dental practitioner in orthodontic or oral surgery),
- in Part Three, documents certifying the acquired rights of formal qualifications (competence) for the profession of pharmacist.

ČÁST PRVÁ

Documents certifying acquired rights of formal qualifications (professional qualifications) for the profession of doctor or formal qualifications for the relevant field of specialisation of doctor (professional) referred to in Directive 93 / 16 / EEC
A Acquisition of the right of formal qualifications (professional competence) to pursue the profession of doctor
A.1 Diplomas, certificates or other evidence of formal qualifications (proficiency) for the pursuit of the profession of medical practitioners of Member States who do not meet all the essential requirements of medical training (1) have been issued by Member States and provide evidence of training initiated by:
- before 1 January 1986 for Spain and Portugal,
- before 1 January 1981 in the case of Greece,
- before 20 December 1976 in the case of the other Member States,
- before the date of accession, in the case of Austria, Finland and Sweden, i.e. 1.1.1995,
- before the date of accession for the Czech Republic, Estonia, Cyprus, Latvia, Lithuania, Hungary, Malta, Poland, Slovenia and Slovakia, i.e. 1.5.2004,
must be accompanied by a certificate certifying that they have pursued the profession of doctor for at least three consecutive years during the five years preceding the date of issue of the certificate. On the basis of these documents, the Czech Republic will recognise acquired rights of formal qualifications (competence) for the profession of doctor.
A.2 Diplomas, certificates or other evidence of qualification (competence) for the pursuit of the profession of medical practitioners of Member States acquired in the territory of the former German Democratic Republic which do not meet all the essential requirements of medical training (1) shall be recognised by the Czech Republic if:
- evidence of training started before German unification, i.e. before 3 October 1990,
- authorise the holder to carry out the activities of a doctor throughout the territory of Germany under the same conditions as the documents issued by the competent German authorities (2), and
- are accompanied by a certificate issued by the competent German authorities showing that they have been practising the profession of doctor in Germany for at least three consecutive years during the five years preceding the date of issue of the certificate.
A.3 Diplomas, certificates or other evidence of qualification (competence) for the pursuit of the profession of medical practitioners of Member States which do not correspond to the designation given for that Member State (2) shall be accompanied by a certificate issued by the competent authorities or bodies certifying that the Member State which issued them considers them to be diplomas with a corresponding designation (i.e. equivalent) (2).
B Acquisition of formal qualifications for the relevant field of medical specialisation (specialist)
B.1 Diplomas, certificates or other evidence of formal qualifications for the relevant field of specialisation of the doctor (specialist) of the members of the Member States who do not meet all the essential requirements of training have been issued by the Member States and provide evidence of training (specialisation training) initiated by:
- before 1 January 1986 for Spain and Portugal,
- before 1 January 1981 in the case of Greece,
- before 20 December 1976 in the case of the other Member States,
- before the date of accession, in the case of Austria, Finland and Sweden, i.e. 1.1.1995,
- before the date of accession for the Czech Republic, Estonia, Cyprus, Latvia, Lithuania, Hungary, Malta, Poland, Slovenia and Slovakia, i.e. 1.5.2004,
they must be accompanied by a certificate issued by the competent authorities or bodies of the Member State of origin or of the last establishment showing the performance of the professional profession for a period equivalent to twice the difference between the length of specialised training in the Member State of origin or last establishment and the minimum length of training set out in Annex 2 to this communication.
However, if, before 1 May 2004, the Czech Republic requested a minimum duration of training shorter than that laid down in Annex 2 to this communication, the difference may be determined only on the basis of this shorter minimum duration of training set out in the Czech Republic before 1 May 2004.
B.2 Diplomas, certificates or other evidence of formal qualifications for the relevant field of specialisation of the doctor (specialist) of the members of the Member States acquired in the territory of the former German Democratic Republic which do not meet all the essential requirements of training shall be recognised by the Czech Republic if:
- provide evidence of training (specialisation training) started before 3 April 1992, and
- authorise the holder to carry out the activities of a specialist doctor throughout the territory of Germany under the same conditions as documents issued by the competent German authorities (3).
The Ministry of Health of the Czech Republic may require an additional certificate issued by the competent German authority or body certifying the performance of the professional medical activities for a period equivalent to twice the difference between the length of specialised training obtained in the territory of Germany and the minimum length of training specified in Annex 2 to this communication.
B.3 Diplomas, certificates or other evidence of qualifications for the relevant field of specialisation of the doctor (expert) of the members of the Member States which do not correspond to the designation given for that Member State (3) shall be accompanied by a certificate issued by the competent authorities or bodies certifying that the Member State which issued them considers them to be diplomas with a corresponding indication (i.e. equivalent) (3).
B.4 Member States which have repealed the laws, regulations and administrative provisions relating to the issue of diplomas, certificates and other evidence of formal qualifications in professional medicine (specialisation) in the field of neuropsychiatry, radiology, cardiothoracic surgery, vascular surgery, gastro-enterological surgery, biological hematology, rehabilitation medicine or tropical medicine, and which have taken measures concerning acquired rights in favour of their own nationals, grant to nationals of other Member States the right to use such qualifications and other evidence of qualifications in areas where their diplomas, certificates and other evidence of formal qualifications in vocational medicine (specialisation) in the fields of neurosychiatry, radiology, cardiothoracic, cardiothoracic surgery, gastro-enterological surgery, biological hematology, rehabilitation or tropical medicine fulfil the conditions laid down in paragraph B.1 or have been subject to the conditions of professional training before the date from which the host Member State of which they have ceased to issue such diplomas such diplomas a certificate, or other evidence of qualifications, and other evidence of qualifications.
The dates from which the Member States concerned repealed the laws, regulations and administrative provisions relating to diplomas, certificates and other evidence of formal qualifications in the fields referred to in the first sentence of paragraph B.4 are set out in Annex 1 to this communication.
B.5 Diplomas, certificates and other evidence of formal qualifications in vocational medicine issued in Spain to doctors who, before 1 January 1995, have completed specialised training which does not meet the essential requirements of training, the Czech Republic recognises as sufficient evidence if the document is accompanied by a certificate issued by the competent Spanish authorities confirming that the person concerned has passed a proficiency test organised under the special regulatory measures contained in Royal Decree 1497 / 99 in order to verify that the person concerned has a level of knowledge and competence comparable to that of the medical practitioners holding the designation of professional practitioner mentioned for Spain in the communication of the Ministry of Health No 316 / 2005 Coll.
C Rights acquired concerning formal qualifications (professional qualifications) for the profession of doctor and formal qualifications for the relevant field of specialisation of doctor (specialist)
C.1 Diplomas, certificates and other evidence of formal qualifications in the field of medicine and professional medicine of Member States which have been awarded to them or whose training began in the former Soviet Union before 20 August 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Estonia confirm that these documents have the same legal validity in their territory as Estonian evidence of formal qualifications in medicine and vocational medicine as regards access to and pursuit of the profession of doctor. This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out the activity in question on the territory of Estonia for at least three consecutive years during the five years preceding the date of issue of the certificate.
C.2 Diplomas, certificates and other evidence of formal qualifications in the field of medicine and professional medicine of Member States awarded to them or whose training began in the former Soviet Union before 21 August 1991, shall be recognised by the Czech Republic as sufficient evidence if the authorities of Latvia confirm that they have the same legal validity in their territory as Latvian evidence of formal qualifications in medicine and professional medicine as regards access to and pursuit of the profession of doctor. This certificate must be accompanied by a certificate issued by the same authorities stating that those nationals of the Member States have actually and lawfully carried out the activity in question in Latvia for at least three consecutive years during the five years preceding the date of issue of the certificate.
C.3 Diplomas, certificates and other evidence of formal qualifications in the field of medicine and professional medicine of Member States which have been awarded to them or whose training began before 11 March 1990 in the former Soviet Union shall be recognised by the Czech Republic as sufficient evidence if the authorities of Lithuania confirm that they have the same legal validity in their territory as Lithuanian evidence of formal qualifications in the field of medicine and vocational medicine as regards access to and pursuit of the profession of doctor. This certificate shall be accompanied by a certificate issued by the same authorities stating that those nationals of the Member States have actually and lawfully carried out the activity in question in Lithuania for at least three consecutive years during the five years preceding the date of issue of the certificate.
C.4 Diplomas, certificates and other evidence of formal qualifications in the field of medicine and vocational medicine of members of the Member States who have been awarded them or whose training began in the former Czechoslovakia before 1 January 1993 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovakia confirm that they have the same legal validity in their territory as Slovak evidence of formal qualifications in the field of medicine and professional medicine as regards access to and pursuit of the profession of doctor. This certificate must be accompanied by a certificate issued by the same authorities indicating that these nationals of the Member States have actually and lawfully carried out the activity in question in Slovakia for at least three consecutive years during the five years preceding the date of issue of the certificate.
C.5 Diplomas, certificates and other evidence of formal qualifications in the field of medicine and professional medicine of the members of the Member States who have been awarded them or whose training began in Yugoslavia before 25 June 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovenia confirm that they have the same legal validity in their territory as Slovenian evidence of formal qualifications in the field of medicine and professional medicine as regards access to and pursuit of the profession of doctor. This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out the activity in question in Slovenia for at least three consecutive years during the five years preceding the date of issue of the certificate.

ČÁST DRUHÁ

Documents certifying acquired rights of formal qualifications (professional competence) for the separate exercise of the profession of dental practitioner or formal qualifications for the relevant field of specialisation of dental practitioner (dental practitioner in orthodontics or oral surgery) referred to in Directives 78 / 686 / EEC and 78 / 687 / EEC
1. Diplomas, certificates or other evidence of formal qualifications (professional qualifications) for the pursuit of the profession of dental practitioner of Member States which do not satisfy all the essential requirements of dental training 1), or of which diplomas, certificates or other evidence of specialised qualifications for the pursuit of the profession of dental practitioner were issued by those Member States before 28.1.1980 and, in the case of Italy before 28.7.1984, they shall submit additional certificates certifying that they have pursued the profession of dental practitioner for at least three consecutive years during the five years preceding the date of issue of the certificate on which each Member State shall recognise the acquired rights of formal qualifications (professional qualifications) for the profession of dental practitioner.
2. Diplomas, certificates or other evidence of formal qualifications for the relevant field of specialisation of the dental practitioner (specialised dental practitioner) of Member States which do not meet all the essential requirements of specialised training, or whose diplomas, certificates or other evidence of formal qualifications (specialised qualifications) for the pursuit of the profession of dental practitioner were issued by those Member States before 28.1.1980 and, in the case of Italy, before 28.7.1984, they shall submit an additional certificate issued by the competent authorities or bodies of the Member State of origin or the last establishment which attests the professional practitioner for a period equivalent to twice the difference between the length of specialised training in the Member State of origin or last establishment and between the three years of minimum training.
3. Diplomas, certificates and other evidence of formal qualifications of dental practitioners or specialised dentists in orthodontics and oral surgery of members of the Member States which do not correspond to the designation given for that Member State (4) shall be recognised by the Czech Republic as sufficient evidence if they have been issued by the Member States and accompanied by a certificate issued by the competent authorities or bodies. This supplementary certificate confirms that these diplomas, certificates and other evidence of formal qualifications of dental practitioners or specialised dental practitioners in orthodonnia and oral surgery attest to training and that the Member State which issued them considers them to be diplomas with an appropriate designation (i.e. equivalent) 4.
4. Diplomas, certificates and other evidence of formal qualifications of dental practitioners of members of the Member States in the territory of the former German Democratic Republic, which does not meet all the essential requirements of training, shall be recognised by the Czech Republic as sufficient evidence if:
- evidence of training started before German unification, i.e. before 3 October 1990,
- authorise the holder to carry out the activities of a dental practitioner throughout the territory of Germany under the same conditions as the documents issued by the competent German authorities (5), and
- are accompanied by a certificate issued by the competent German authorities showing that the members of the Member States have actually and lawfully carried out the activities in question in Germany for at least three consecutive years during the five years preceding the date of issue of the certificate.
5. Diplomas, certificates and other evidence of formal qualifications of professional dentists of members of the Member States in the territory of the former German Democratic Republic, which does not meet all the essential requirements of training, shall be recognised by the Czech Republic as sufficient evidence if:
- evidence of training started before German unification, i.e. before 3 October 1990,
- authorise the holder to carry out the activities of a dental practitioner throughout the territory of Germany under the same conditions as documents issued by the competent German authorities (6).
The Ministry of Health of the Czech Republic may require such diplomas, certificates or other evidence of formal qualifications to be accompanied by a certificate issued by the competent German authorities or bodies proving the performance of the professional dental practitioner's activities for a period equivalent to twice the difference between the length of specialised training obtained in the territory of Germany and the minimum duration of training, provided that this period is below the minimum duration of training.
6. Diplomas, certificates and other evidence of formal qualifications in the field of dentistry and professional dentistry of members of the Member States who were awarded or whose training began in the former Soviet Union before 20 August 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Estonia confirm that these documents have the same legal validity in their territory as Estonian evidence of formal qualifications in dental medicine and professional dentistry as regards access to and pursuit of the profession of dental practitioner. This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out the activity in question on the territory of Estonia for at least three consecutive years during the five years preceding the date of issue of the certificate.
7. Diplomas, certificates and other evidence of formal qualifications in the field of dentistry and professional dentistry of members of the Member States who were awarded or whose training began in the former Soviet Union before 21 August 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Latvia confirm that such qualifications have the same legal validity in their territory as Latvian evidence of formal qualifications in dental medicine and professional dental medicine as regards access to and pursuit of the profession of dental practitioner. This certificate must be accompanied by a certificate issued by the same authorities stating that those nationals of the Member States have actually and lawfully carried out the activity in question in Latvia for at least three consecutive years during the five years preceding the date of issue of the certificate.
8. Diplomas, certificates and other evidence of formal qualifications in the field of dentistry and professional dentistry of members of the Member States who have been awarded or started training in the former Soviet Union before 11 March 1990 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Lithuania confirm that these documents have the same legal validity in their territory as Lithuanian evidence of formal qualifications in dental medicine and professional dentistry as regards access to and pursuit of the profession of dental practitioner. This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out the activity in question in Lithuania for at least three consecutive years during the five years preceding the date of issue of the certificate.
9. Diplomas, certificates and other evidence of formal qualifications in the field of dentistry or professional dentistry of members of the Member States who were awarded or whose training began in Yugoslavia before 25 June 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovenia confirm that these documents have the same legal validity on their territory as Slovenian evidence of formal qualifications in dental medicine and professional dentistry as regards access to and pursuit of the profession of dental practitioner. This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out the activity in question in Slovenia for at least three consecutive years during the five years preceding the date of issue of the certificate.
10. Diplomas, certificates and other documents issued in Italy concerning formal qualifications in the field of medicine of members of the Member States who have started higher education in medicine no later than 28 January 1980 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Italy confirm that these documents have the same legal validity in their territory as Italian evidence of formal qualifications in dental medicine as regards access to and pursuit of the profession of dental practitioner. This certificate must be accompanied by a certificate issued by the same authorities indicating that those nationals of the Member States have actually and lawfully carried out the activity in question in Italy for at least three consecutive years during the five years preceding the date of issue of the certificate. The requirement of three years of experience referred to in this paragraph shall not apply to persons who have successfully completed at least three years of training, which the competent authorities of Italy have recognised as equivalent to dental training.
Diplomas, certificates and other documents issued in Italy concerning formal qualifications in medicine of members of the Member States who began higher education studies of medicine between 28 January 1980 and 31 December 1984 shall be recognised by the Czech Republic as sufficient evidence if they are accompanied by a certificate issued by the competent Italian authorities stating that:
- these persons have completed a professional qualification test with the competent Italian authority to demonstrate that they have reached the level of knowledge and skills as those holding the markings listed for Italy in the Communication of the Ministry of Health No. 316 / 2005 Coll.,
- those persons in Italy, during the five years prior to the issue of the certificate, have actually carried out the activities of dental practitioner as a principal activity for at least three consecutive years,
- these persons are entitled to exercise or actually perform the activities of dental practitioner in accordance with the law under the same conditions as the holders of the markings listed for Italy in the Communication of the Ministry of Health No. 316 / 2005 Coll.
The requirement to pass the proficiency test referred to in the first subparagraph of the preceding paragraph may be waived in the case of persons who have successfully completed at least three years of study declared equivalent to dental training by the competent authorities of Italy.
11. Diplomas, certificates and other documents issued in Slovakia or in former Czechoslovakia prior to 1 January 1993 concerning formal qualifications in the field of medicine of members of the Member States who began higher education studies of medicine before 1 May 2004 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovakia confirm that these documents have the same legal validity in their territory as Slovak evidence of formal qualifications in dental medicine as regards access to and pursuit of the profession of dental practitioner. This certificate must be accompanied by a certificate issued by the same authorities indicating that these nationals of the Member States have actually and lawfully carried out the activity in question in Slovakia for at least three consecutive years during the five years preceding the date of issue of the certificate. The requirement of three years of experience referred to in this paragraph shall be waived in the case of persons who have successfully completed at least three years of study, certified by the competent authorities as equivalent to dental training.
12. Diplomas, certificates and other documents issued in Spain concerning formal qualifications in the field of medicine of members of the Member States who began their university studies of medicine before 1 January 1986 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Spain confirm that these documents have the same legal validity in their territory as Spanish evidence of formal qualifications in dental medicine as regards access to and pursuit of the profession of dental practitioner. This certificate shall be accompanied by a certificate issued by the same authorities indicating that those nationals of the Member States have actually and lawfully carried out the activity in question in Spain for at least three consecutive years during the five years preceding the date of issue of the certificate. The requirement of three years' experience referred to in this paragraph shall not apply to persons who have successfully completed at least three years' training, which the competent authorities of Spain have recognised as equivalent to that of a dental practitioner.
13. Diplomas, certificates and other documents issued in Austria concerning formal qualifications in the field of medicine of members of the Member States who began higher education studies of medicine before 1 January 1994 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Austria confirm that these documents have the same legal validity in their territory as Austrian evidence of formal qualifications in dental medicine as regards access to and pursuit of the profession of dental practitioner. This certificate shall be accompanied by a certificate issued by the same authorities indicating that these nationals of the Member States have actually and lawfully carried out the activity in question on the territory of Austria for at least three consecutive years during the five years preceding the date of issue of the certificate. The requirement of three years' experience referred to in this paragraph shall not apply to persons who have successfully completed at least three years' training, which the competent authorities of Austria have recognised as equivalent to that of a dental practitioner.

ČÁST TŘETÍ

Documents certifying the acquired rights of formal qualifications (competence) for the profession of pharmacist referred to in Directives 85 / 432 / EEC and 85 / 433 / EEC
1. Diplomas, certificates and other university or equivalent evidence of formal qualifications in the field of pharmacy of members of the Member States, issued by Member States which do not meet all the essential requirements for training (1), shall be considered as satisfying the following requirements:
- attest to education completed before 1 October 1987,
or
- attest to education completed after 1 October 1987 but started before 1 October 1987,
and in both cases,
- if they are accompanied by a certificate certifying that their holders have actually and lawfully carried out one of the activities referred to in Law 7 in a Member State) for at least three consecutive years during the five years preceding the date of issue of the certificate and provided that such activity is regulated by the State concerned.
2. Diplomas, certificates and other evidence of university or similar qualifications in the field of pharmacy of members of the Member States, issued by Member States which meet all the essential requirements of vocational training (1), but which do not comply with the designation given in the Ministry of Healthcare Communication (8), shall be deemed to correspond to the diplomas mentioned in the Ministry of Health Communication (9), provided that they are accompanied by a certificate documenting the training and the Member State which issued them considers them to be diplomas with the corresponding marks9).
3. Diplomas, certificates and other evidence of higher education or similar qualifications in the field of pharmacy of members of the Member States, which attest to training in the territory of the former German Democratic Republic which does not meet all the essential requirements of the vocational preparation (1), shall be recognised by the Czech Republic as sufficient evidence if:
- evidence of training started before German unification, i.e. before 3 October 1990,
- entitle the holder to carry out the activities of a pharmacist throughout the territory of Germany under the same conditions as the documents issued by the competent German authorities (9), and
- are accompanied by a certificate issued by the competent German authorities showing that the members of the Member States have actually and lawfully carried out one of the activities referred to in Law 7) in Germany for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Germany.
4. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States, which have been awarded or started training in the former Soviet Union before 20 August 1991, shall be recognised by the Czech Republic as sufficient evidence if the authorities of Estonia confirm that these documents have the same legal validity in their territory as Estonian evidence of formal qualifications in the field of pharmacy as regards access to, and performance of, business). This certificate must be accompanied by a certificate issued by the same authorities stating that such nationals of the Member States have actually and lawfully carried out one of the activities (7) on the territory of Estonia for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Estonia.
5. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States, which have been awarded or started training in the former Soviet Union before 21 August 1991, shall be recognised by the Czech Republic as sufficient evidence if the authorities of Latvia confirm that these documents have the same legal validity in their territory as Latvian evidence of formal qualifications in the field of pharmacy as regards access to, and performance of, action. This certificate must be accompanied by a certificate issued by the same authorities indicating that these nationals of the Member States have actually and lawfully carried out one of the activities (7) on the territory of Latvia for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Latvia.
6. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States, which have been awarded or started training in the former Soviet Union before 11 March 1990, shall be recognised by the Czech Republic as sufficient evidence if the authorities of Lithuania confirm that they have the same legal validity in their territory as Lithuanian evidence of formal qualifications in the field of pharmacy as regards access to, and performance of, business. This certificate must be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out one of the activities (7) in Lithuania for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Lithuania.
7. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States who have been awarded or whose training began in the former Czechoslovakia before 1 January 1993 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovakia confirm that they have the same legal validity in their territory as Slovak evidence of formal qualifications in the field of pharmacy in terms of access to, and performance of, business. This certificate must be accompanied by a certificate issued by the same authorities indicating that such nationals of the Member States have actually and lawfully carried out one of the activities (7) in Slovakia for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Slovakia.
8. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States who were awarded or whose training began in Yugoslavia before 25 June 1991 shall be recognised by the Czech Republic as sufficient evidence if the authorities of Slovenia confirm that these documents have the same legal validity in their territory as Slovenian evidence of formal qualifications in the field of pharmacy in terms of access to and performance of the activity). This certificate shall be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out one of the activities (7) on the territory of Slovenia for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Slovenia.
9. Diplomas, certificates and other evidence of formal qualifications in the field of pharmacy of members of the Member States, which were awarded in Italy after completion of the training started before 1 November 1993 and completed before 1 November 2003, shall be recognised by the Czech Republic as sufficient evidence if the authorities of Italy confirm that they have the same legal validity in their territory as Italian evidence of formal qualifications in the field of pharmacy as regards access to, and enforcement of, business. This certificate must be accompanied by a certificate issued by the same authorities stating that these nationals of the Member States have actually and lawfully carried out one of the activities (7) in Italy for at least three consecutive years during the five years preceding the date of issue of the certificate, provided that such activity is regulated in Italy.
