DECLARATION
of 30 August 2005
amending Decree No 589 / 2004 Coll., on the reimbursement of medicinal products and foodstuffs for special medical purposes, as amended by Decree No 225 / 2005 Coll.
The Ministry of Health provides pursuant to § 15 paragraph 10 of Act No. 48 / 1997 Coll., on Public Health Insurance and on the amendment and addition of certain related laws, as amended by Act No. 438 / 2004 Coll.:
Decree No 589 / 2004 Coll., on the reimbursement of medicinal products and foodstuffs for special medical purposes, as amended by Decree No 225 / 2005 Coll., is amended as follows:
1. In Annex 1 to the Decree, row 31.1 in the column "Reimbursement in CZK 'is replaced by" 4.06' by "5.28 '.
2. In Annex No 1 to the Decree, row 82.5 in the column "Reimbursement in CZK 'is replaced by" 8.69' by "8.68 '.
3. In Annex 1 to the Decree, in row 82.7, in the column "name of active substance ', the words" containing 12,5 mg' are added after the words "and the diuretic '.
4. In Annex 1 to the Decree, the following row 82.8 is inserted after row 82.7:

The current rows 82.8 to 82.20 are renumbered as rows 82.9 to 82.21.
5. In Annex 1 to the Decree, row 82.12 in the column "Reimbursement in CZK 'is replaced by" 8.87' by "12.74 '.
6. In Annex 1 to the Decree, row 82.14 in the column "Reimbursement in CZK 'is replaced by" 6.47' with "6.00 '.
7. In Annex No 1 to the Decree, row 82.15 in the column "Reimbursement in CZK 'is replaced by" 10.07' by "8.21 '.
8. In Annex 1 to the Decree, row 82.16 in the column "Reimbursement in CZK 'is replaced by" 10.54' by "8.68 '.
9. In Annex 1 to the Decree, row 82.17 in the column "Reimbursement in CZK 'is replaced by" 6.47' by "6.00 '.
10. In Annex No 1 to the Decree, row 82.19 in the column "Reimbursement in CZK 'is replaced by" 6.47' by "6.00 '.
11. In Annex 1 to the Decree, row 109.6 in the column "submission 'is replaced by" trando' by "loc '.
12. In Annex 1 to the Decree, row 109.6 in the column "quantity 'is replaced by" 0.30' by "0.045 '.
13. In Annex 1 to the Decree, in the sentence of the first text under row 1113, the word "tolterodine 'is deleted.
14. In Annex 1 to the Decree, in row 142.12, in the column "quantity ', the number" 0.50' is replaced by "0.125 '.
15. In Annex 1 to the Decree, row 143.16 in the column "Restriction ', the letter" P' is replaced by the letter "X '.
16. In Annex 1 to the Decree, in row 225.8 in the column "Restriction ', the words" L / PSY, SEX, NEU' are deleted.
17. In Annex 1 to the Decree, row 253.3 in the column "Payment 'is replaced by" 73.91' by "86.43 '.
18. In Annex 1 to the Decree, the sentence "The therapy is carried out by a specialised nuclear medicine centre following the haematooncology centre."
19. In Annex 1 to the Decree, the words:
