DECLARATION

on the model of the standardised social worker record
The Ministry of Labour and Social Affairs provides for the Act No. 73 / 2011 Coll., on the Labour Office of the Czech Republic and on the amendment of related laws, as amended by Act No. 366 / 2011 Coll., Act No. 401 / 2012 Coll. and Act No. 306 / 2013 Coll.:
The model of the Standardised Record of a Social Worker is set out in the Annex to this Order.
This Decree shall take effect on 1 November 2013.
