laying down the model of the questionnaire, the models of the military documents and their handling and the model of the mark and the rules governing its wearing
The Ministry of Defence provides pursuant to § 19 (6) and § 33e of Act No. 585 / 2004 Coll., on the defence obligation and its provision (the Defence Act), as amended by Act No. 47 / 2016 Coll.:
Subject matter
This decree sets out the model of the questionnaire used to obtain basic data on the citizen for the purposes of the levy procedure ("the questionnaire '), the model of the military documents and their handling, and the model of the mark and the rules of its wearing.
Questionnaire
The model of the questionnaire is set out in Annex 1 to this order.
Military ID
(1) The military licence is printed on a white plastic card of format CR- 80 with dimensions 54 mm x 85,6 mm. The background of the front of the military licence consists of a white background with the contours of a two-tail lion in a jump made in grey with yellow details. On the front of the military pass in the upper left corner is a colour photograph showing the face of a soldier. The title of the licence, the designation of nationality of the holder according to the international standards by the code CZE and the particulars provided for by the law on defence. On the back of the military licence are the instructions for surrender of the military licence in case of its detection, in the Czech language in the upper part and in the English language in the lower part. The language versions of the instructions may not be the same. In addition, information on the issue of a military licence in English in accordance with the Geneva Convention for the Protection of the Victims of War is given on the back of the enclosed text field.
(2) The model of the military licence is set out in Annex 2 to this Decree.
Military book
(1) A military book is a military document of the type of book. It has a grey plastic packaging with dimensions 115 mm x 88 mm and 52 pages with a sheet size 108 mm x 83 mm. On the first page of the military book is a lesson for the holder and instructions for the finder in Czech and English. The language versions of the instructions may not be the same. On the third side at the top there is a colour photograph showing the face of a soldier. In the military book, data on the holder and other data shall be recorded to the extent provided for in the Law of Defense.
(2) The model of the military book is set out in Annex 3 to this Decree.
Application order
The models of call orders are set out in Annexes 4 to 6 to this Decree.
Handling of military documents
A soldier with military documents shall be treated in such a way as to avoid damage, loss, theft, abuse or destruction.
Personal mark and rules on its wearing
(1) A personal mark is a metal slice of rectangular shape with a thickness of 0,5 mm to 1,5 mm with rounded corners and defined identification details of the soldier. A set of personal marks is issued to the soldier, which consists of 2 identical pieces of the personal mark, 1 piece of a 685 mm long hanging chain, 1 piece of a 140 mm long hanging chain and 2 pieces of a protector. The mark and the necklace are made of stainless, refractory, refractory and anti-allergic steel, enabling them to be sterilised and decontaminated. Both personal marks are provided with a protective device made of non-toxic silicone rubber, which does not cause allergic reactions and is resistant to the decontamination process.
(2) The following data on the soldier shall be recorded in the metal plate in capitals without diacritics in inseparable manner:
(a) surname and surname; other surnames or names shall be given in abbreviation after the surname or by name, if they cannot be given in full on the personal mark,
(b) a personal number,
(c) the designation of nationality under international standards by code CZE; and
(d) blood type.
(3) A soldier in active duty carries a military stamp in a set, in such a way that the chains are joined and 1 piece of the personal mark is hung on each of them. A soldier in active duty outside a state of danger or a state of war shall carry a set together unless the service body decides to carry it around his neck.
(4) The model of the mark is set out in Annex 7 to this Order.
Repeal
Decree No. 102 / 2005 Coll., laying down the model of the questionnaire and models of military documents, is hereby repealed.
Efficacy
This decree shall take effect on the day of its publication.
