laying down hygiene requirements for the handling of laundry and for the washing of laundry from medical devices
The Ministry of Health of the Czech Republic provides pursuant to § 70 paragraph 1 (c) and § 71 paragraph 2 (a) of Act No. 20 / 1966 Coll., on the care of the health of the people, as amended by the Act of the Czech National Council No. 548 / 1991 Coll. (full text with effect for the Czech Republic No. 86 / 1992 Coll.):
§ 1
Preliminary provisions
(1) For the purposes of this Decree, linens and personal linens used by patients, personal protective equipment of doctors, nurses and other healthcare professionals, surgical linens, diapers, towels, kitchen linens and bedlinen used by medical professionals shall be understood as such.
(2) From the point of view of health risk, for the purposes of this Decree, laundry shall be divided into:
(a) highly infectious linens - linens used in the treatment of persons who are ill or suspected of being sick by plague, cholera, yellow fever;
(b) infectious linens - linens used in infectious and tuberculosis and respiratory disorders, in hemodialysis units, in microbiological and biochemical laboratories and in pathology;
(c) laundry of polluted cytostatics - laundry used in the preparation and application of cytostatic solutions,
(d) laundry of contaminated radionuclide - laundry used in the investigation and treatment of radionuclides,
(e) other linen for which suspicion of infection cannot be excluded.
Sorting and packing of laundry
§ 2
(1) The linen shall be stored in the departments, halls or other premises (hereinafter referred to as "separation ') in packages marked with a label or stripe (hereinafter referred to as" packaging') for:
(a) highly infectious linen - red,
(b) infectious linens - yellow,
(c) laundry of polluted cytostatics - black,
(d) laundry contaminated with radionuclides - blue or marked "R,"
(e) laundry suspected of being infected - white or unmarked.
(2) Before storing in the packaging on the compartments, the laundry shall not be shaken.
(3) In packaging, the laundry shall be stored separately by species and degree of pollution, namely:
(a) patients' bed and personal linen, white and stable, slightly soiled,
(b) patients' bed linen and personal linen, white and durable medium soiled,
(c) bedlinen and personal linen of patients, white and stable, heavily soiled,
(d) personal protective equipment of doctors and nurses,
(e) personal protective equipment of other health workers;
(f) bedlinen for medical workers,
g) bed linen and personal linen of newborns,
(h) diapers,
(i) operating linen and birthing linen,
(j) laundry used in the kitchen,
(k) any other permanent dyeing.
§ 3
(1) The packaging in which the laundry is stored must be impermeable, easily closed and suitable for washing. Packaging which cannot be washed must be washable and disinfected or for single use.
(2) Packaging with used laundry is stored in a dedicated space with direct ventilation. The floor and walls of this space shall be washable at least up to 150 cm and disinfected daily.
§ 4
When handling used laundry, workers shall use dedicated personal protective equipment (protective clothing, hat, protective mask and gloves) and comply with the principles of personal hygiene (1).
§ 5
(1) Used laundry is transported to the laundry room daily, only in the covered wagon reserved for these purposes. The loading area of the vehicle is disinfected daily. If the laundry is transported in containers, the containers must be disinfected daily.
(2) Polished radionuclide laundry is weighed down to a level where the surface activity of the laundry is less than 300 kBq.m-2 and, for alpha lamps, less than 30 kBq.m-2 and where the increase in dose equivalent power in 1 m from the packaging surface is less than 0,1 µsv.h-1. The reduction of contamination takes place directly in the nuclear medicine departments by repeatedly pre-pressing the automatic washing machine or by storing it in a separate room for the necessary time.
Sanitary requirements for laundry operation
§ 6
(1) Highly infectious laundry can only be washed in the laundry facilities. Highly infectious linens shall not leave the compartment without effective decontamination. Decontamination of laundry outside the premises of the department shall exceptionally be authorised by the competent authority of the sanitary service.
(2) The laundry provided in labelled packages must not be sorted in the laundry room.
(3) The laundry is inserted into machines so that the operator is not exposed to the risk of infection and its direct contact with the laundry is excluded. The laundry must not be shaken.
(4) When working the process and handling the laundry in the laundry room, the laundry used with clean laundry shall not be crossed.
§ 7
(1) The laundry is disinfected while working the process. Infectious laundry should be washed thermodesosamically or chemothermodesosamically. It can only be washed in centrifuge washing machines with a dividing wall separating the dirty and clean side. Where there is no central disinfecting of waste water, the water used in the first stage of the working process must be disinfected in the laundry before discharge into the sewage. The laundry of polluted cytostatics is washed with thermodesinfectant.
(2) The thermo-infectious treatment is carried out at a water temperature of 90 oC for 10 minutes after reaching this temperature.
(3) The chemodisinfectant process is either:
(a) the process with peracids or perborates as an active substance at a water temperature of 60 oC for a period of 20 minutes after reaching that temperature; or
(b) a process with chlorine, organic or inorganic derivatives containing active chlorine at a water temperature of 60 oC for 20 minutes after reaching that temperature.
(4) The laundry suspected of infection is washed in continuous washing machines. The disinfectant process must be completed before the waving phase and the bath must not be used upstream before the end of the disinfection. Water from the draining press may only be used for the first process zone (wetter and pre-wash).
(5) The technological process of the washing process is chosen by type of laundry, degree of pollution, type of washing machine and type of washing products used. The concentration of detergents is determined by their species.
§ 8
(1) After washing, the wash is ironed at a temperature of at least 150 oC. This requirement is met even if the laundry is dried at a temperature of not less than 140 oC during drying.
(2) During dispatch, the laundry shall be packed in such a way as to prevent its contamination with pathogens. It is stored in washed bags or closed plastic packaging.
(3) The laundry is delivered in covered carriages for that purpose, which are cleaned and disinfected daily.
§ 9
A mandatory assessment by the competent authority of the sanitary service.2) is a condition for starting the operation of laundry detergents from medical facilities.
§ 10
Efficacy
This decree shall take effect on the day of its publication.
