amending Decree No. 134 / 1998 Coll., issuing a list of health performance with points, as amended

The Ministry of Health provides pursuant to § 17 (4) of Act No. 48 / 1997 Coll., on Public Health Insurance and on the amendment and addition of certain related laws, as amended by Act No. 371 / 2021 Coll.:

1. In the Annex to Chapter 1, at the end of the table, the following point 19 is added:

„19.

centrum vysoce specializované péče

poskytovatel, který je podle zákona o zdravotních službách držitelem statutu centra vysoce specializované zdravotní péče nebo centra vysoce specializované zdravotní péče pro pacienty se vzácným onemocněním, nebo spolupracujícím poskytovatelem s takovým poskytovatelem pro účely udělení nebo získání tohoto statutu“.

2. In the Annex to Chapter 2, point 6.2.1 of the Introductory Part of the provision, "3 'is replaced by" 2' and point 2 is deleted.

Point 3 is renumbered point 2.

3. In the Annex to Chapter 2, point 6.3.2 (2), the words "direction 'are deleted.

4. In Chapter 2 (6.5) (a) of the Annex, "3,0616 'is replaced by" 3,5239', "1,8237 'is replaced by" 2,0991' and "2,4619 'is replaced by" 2,8336'.

5. In the second sentence of point 8.1 of the Annex to Chapter 2, the words "or the insured person has been declared in the course of 24 hours prior to admission to hospital with that provider" shall be inserted after the word "expertise."

6. In the Annex, at the end of Chapter 2, the following point 16 is added:

For the treatment of the child, the output shall be Nos 09555, 09556 and 09557. Performance shall be reported with performance that has been provided to the child by the date of 18 years in his personal presence.

Performance shall be reported with clinical examination performance; for the reporting of the performance Nos 09555, 09556 and 09557, for the expertise 001 and 002, for the performance of clinical examinations covered by the Decree pursuant to Article 17 (5) of Act No. 48 / 1997 Coll., on Public Health Insurance and on the amendment and addition of certain related laws, as amended, no current reporting of such performance of clinical examinations is required until the surrender payment. In addition, performance shall be reported with expert performance not having established clinical examinations, i.e. competence 719, 780, 697, 809, 810, 902, 905, 910, 913, 916, 917, 919, 925, 926 and 927.

In one day one insured person may be reported one performance No 09555, 09556 or 09557 in one professional capacity. If a child requires multiple examinations within one day, performance may be reported for each additional expertise.

Furthermore, the performance of the child under general anesthesia and the performance of the expertise 014, 015, 106, 801, 802, 807, 808, 812, 813, 814, 815, 816, 817, 818, 819, 823 and 921 cannot be reported.

Performance Nos 09555, 09556 and 09557 may not be reported with output No:

The performance of psychotherapy can be combined with the performance of clinical examinations in the provision of care in mental health centres and with the provider of medical services in an ambulance with extended care without restriction, only clinical examinations and performance for group psychotherapy can be combined in ambulances of competence 305 and 901. '

8. '

18. In the Annex to Chapter 101 - Internal medicine - Intern of Performance No 11511, 11512 and 11513:

	Zaučení diabetika v samostatné péči o nohy je indikováno pacientům u poskytovatele zdravotních služeb poskytujícího zdravotní služby v rámci podiatrické ambulance, ve které jsou léčeni s aktivním syndromem diabetické nohy (ulcerací nebo gangrénou na nohou, se známkami infekce včetně osteomyelitidy, flegmóny, stavy po chirurgických zákrocích na nohou, s aktivní Charcotovou osteoarthropatií). Jeho hlavním obsahem je zaučení pacienta a případně i pečujících osob v samostatné péči o ránu a ve správné hygieně (včetně řádné samostatné péče o kůži a nehty) mezi návštěvami lékaře, v adekvátním používání odlehčujících pomůcek, v ochraně druhostranné končetiny a v měření kožní teploty na nohou (například u aktivní Charcotovy ostearthropatie). V případě potřeby je pacient seznámen se známkami preulcerozních lezí a infekce nohou nebo známkách ICHDK nebo její progrese, se symptomy akutní tepenné ischémie, vzniku nebo progrese Charcotovy osteoarthropatie, patologických zlomenin, s preventivním procvičováním nohou a fyzickou aktivitou prováděnou bez zatížení nohou. Výkon lze vykázat u dg. U96.74. Nelze vykazovat s výkony č.13028, 13051, 13053, 06145 a 09523.

	Výkon je indikován pro těhotnou ženu, která byla odeslána do ambulance endokrinologa na základě pozitivního výsledku tyreoidálního screeningu (dvoufázového tyreoidálního testu). Zahrnuje komplexní endokrinologické vyšetření k diagnostice tyreoidálního onemocnění a navržení léčebného plánu. Dále zahrnuje poučení o významu léčby onemocnění v těhotenství včetně seznámení s riziky neléčeného či špatně léčeného onemocnění pro plod a matku. Výkon lze vykázat pouze jednou během těhotenství. Výkon je vykazován pouze u těhotných žen bez aktivní léčby pro thyreopatii. Výkon nelze vykázat současně s výkony č. 14021, 14022, 14023 a 09523. Výkon nelze vykázat současně s výkony klinických vyšetření odbornosti 103.

16101	VYHODNOCENÍ SYNDROMU KŘEHKOSTI (FRAILTY) U GERIATRICKÉHO PACIENTA
	Výkon je určen pro vyhodnocení frailty syndromu u geriatrického polymorbidního pacienta ohroženého zhoršeným funkčním stavem. Cílem je provést diagnostiku geriatrické křehkosti, stanovit její závažnost a míru rizika jejího vzniku či progrese. Dále je součástí výkonu vyšetření jednotlivých domén syndromu frailty podle mezinárodních doporučených postupů a vyhodnotit možnosti jejich intervence. Realizuje geriatr, který po zhodnocení frailty vytvoří individuální terapeutický plán další péče. Je možné ho vykázat jedině s komplexním nebo cíleným či kontrolním vyšetřením geriatrem, aby mohl být na podkladě identifikace příčinných faktorů stanoven individuální plán intervence frailty.
