DECLARATION
of 21 September 2016
amending Decree No. 39 / 2012 Coll., on Dispensary Care
The Ministry of Health provides, pursuant to Section 120 of Act No. 372 / 2011 Coll., on health services and the conditions for their provision (Health Services Act), for the implementation of Section 5 (3) (b) of the Health Services Act:
Decree No. 39 / 2012 Coll., on Dispensary Care, is amended as follows:
1. In the Annex, the following rows 52 to 55 are added at the end of the entry "Provider in general medical practice or other provider ':
| „52 | Chronické onemocnění ledvin ve stádiích CKD 4 a 5 eGFR menší než 0,5 ml/s |
| 53 | Proteinurie větší než 1g/24 hod a mikroskopická hematurie renálního původu |
| 54 | Chronická glomerulonefritis, autosomálně dominantní polycystická degenerace ledvin, vrozená onemocnění ledvin |
| 55 | Stavy po cévní mozkové příhodě“. |
2. In the Annex, the entry "Provider of psychiatry 'is replaced by the following:
| „Poskytovatel v oboru psychiatrie | |
| 01 | Syndrom závislosti |
| 02 | Poruchy nálady středně těžké a těžké, včetně bipolární afektivní poruchy |
| 03 | Schizofrenie, schizoafektivní poruchy, schizotypní porucha, poruchy s bludy |
| 04 | Obsedantně kompulzivní porucha s těžkým průběhem |
| 05 | Posttraumatická stresová porucha |
| 06 | Poruchy sexuální preference včetně stavů po terapeutické kastraci |
| 07 | Poruchy příjmu potravy |
| 08 | Těžké poruchy osobnosti“. |
3. In the Annex, the following entry is inserted after the entry "Provider in the field of psychiatry ':
| „Poskytovatel v oboru dětská a dorostová psychiatrie | |
| 01 | Syndrom závislosti |
| 02 | Poruchy nálady středně těžké a těžké, včetně bipolární afektivní poruchy |
| 03 | Schizofrenie, schizoafektivní poruchy, schizotypní porucha, poruchy s bludy |
| 04 | Obsedantně kompulzivní porucha s těžkým průběhem |
| 05 | Poruchy sexuálního vývoje včetně parafilie |
| 06 | Poruchy příjmu potravy |
| 07 | Poruchy autistického spektra |
| 08 | Těžká chronická tiková porucha - syndrom de la Tourette |
| 09 | Výše neuvedené duševní poruchy dětského a dorostového věku s těžkým disruptivním chováním“. |
Efficacy
This Decision shall enter into force on 17 October 2016.
