DECLARATION
State Office for Nuclear Security
of 13 June 2002
amending Decree No 145 / 1997 of the State Office for Nuclear Safety Coll., on the Registration and Control of Nuclear Materials and on its Closer Definition
According to § 47 (7) for the implementation of § 2 (j), § 4 (16) and (17) and § 18 (1) (b) of Act No. 18 / 1997 Coll., on the Peaceful Use of Nuclear Energy and Ionising Radiation (Atomic Act), and amending and supplementing certain laws, as amended by Act No. 13 / 2002 Coll., hereinafter referred to as "the Act":
Decree No. 145 / 1997 Coll., on the Registration and Control of Nuclear Materials and on its Closer Definition, is amended as follows:
1. Paragraph 1 (1) reads as follows:
"(1) This decree provides:
(a) values for the definition of nuclear materials and the way in which they are recorded and checked;
(b) the way in which the records of uranium and thorium ores are kept and transmitted;
(c) the manner and extent of notification of the management of selected items (1), (2) and the conduct of research and development activities relating to the nuclear fuel cycle. ';
2. In Article 4 (1), the words "including their situation drawings' shall be inserted after the words" data on the construction of nuclear installations'.
3. In Article 4, the following sentence is added at the end of paragraph 1: "If not for a nuclear installation, the holder of the authorisation shall transmit a situation diagram to the Office of the place where the nuclear materials are handled, including a description of the individual buildings and their destination. '.
4. In Article 8 (5), the words "and the term 'shall be inserted after the words" the method of which'.

METHOD OF LEADING AND TRANSFERING OF EVIDENT DATA ON THE LIKE OF Uranium AND THORIUM ORES
Registration details
(1) The evidence of uranium and thorium ore mining and processing is:
(a) identification data, (3) places of uranium mines, modified uranium and plants for the manufacture of thorium concentrate, their operating status and estimated annual capacity;
(b) the quantity of ore extracted, stored, processed and dispatched in tonnes, including the average uranium and thorium content in ore;
(c) the date of dispatch, the addressee, the purpose of the recovery and the quantity of ore in each consignment sent, including the average uranium and thorium content in the ore.
(2) Where the accounting information referred to in paragraph 1 is kept, the keeping of records of nuclear materials pursuant to Sections 3 to 15 of this Decree shall not be required for such uranium and thorium ores.
Transmission of registration data
Registration data shall be transmitted to the Office:
(a) in accordance with Article 15a (1) (a), at least six weeks before the extraction or processing of ore,
(b) in accordance with Article 15a (1) (b), by 15 January each year for the preceding calendar year,
(c) in accordance with Article 15a (1) (c), on form No 8 of the Annex, within five days following the dispatch of the ore.
SCOPE AND METHOD OF TRANSMISSION OF NOTIFICATION OF CERTAIN ACTIVITIES
Transmission of activity data
(1) At least six weeks before the start of the management of selected items (1), (2) and before the start of the research or development activity relating to the nuclear fuel cycle, the identification data, location and scope of the activities carried out shall be transmitted to the Authority.
(2) A 10-year plan shall be submitted to the Authority when research and development activities related to the nuclear fuel cycle are initiated. The update of these plans shall be submitted to the Authority by 31 March of each calendar year. ';
Part Three shall be renumbered PART FIVE.
6.
Archiving documents
The holder of the authorisation shall keep records of the documents referred to in Articles 4 (1), 15a (1) and 15c of this Order for at least five years from their inception. '
7. In Paragraph 18, the words "the register may be reasonably simpler 'are replaced by the words" the register may be simpler with the Authority's consent'.
8. The annex to the Order reads:

Procedures for recording and controlling nuclear materials and for recording uranium and thorium ores
I. Form 1 - Inventory Change Report
When transferring nuclear materials in the Czech Republic, the sender fills out the whole left side of the form and on the right side only "Name and address of the consignee or the code of the installation and the material balance area (MBA)." It shall complete a total of 5 copies, one of which shall be sent to the Office, three of which shall be kept for its own use. The consignee shall complete the right side of the form, send one copy back to the consignor, send one to the Office and keep one for his own use.
In the case of a shipment of nuclear materials abroad or income from abroad, the form shall be completed in two copies. The consignor or consignee shall send one copy to the Office and keep the other for his own use. On consignment abroad, the consignor shall complete the whole left side of the form, on the right side only "Name and address of the consignee or device code and MBA 'and shall equip each consignment abroad with an accompanying document for the foreign consignment in the form of" Export-Import Notification'.
When receiving from abroad, the beneficiary only fills in "Batch data" on the left side and "Name and address of the consignor or device code and MBA" and the whole right side. If the accompanying documentation of the consignor does not indicate "Name of the lot ', this shall be indicated in the note.
In the case of inventory changes made to the authorisation holder (see specifications below), the authorisation holder shall fill in only the left side of the form in two copies. One copy shall be sent to the Office and the other shall be kept for its own use.
The two sections on the top right of the form are not completed, they are used for the purposes of the Office.
A. Batch data
1. "Dose name": The batch name must be made from large letters, numbers, dash, slash, dots and must not contain more than eight symbols. In hand-written codes, the letter "O 'must be crossed out in order to obtain a distinction from the number" 0'.
2. "Material description": Four so-called keywords are used to describe the material:

In the case of a levy containing two or more categories of nuclear material, the names or codes of all the categories in the batch shall be given in the order of their weight data.
4. "Number of items per batch": The number of the same or similar items on which the dose is composed shall be indicated. For example, number of barrels, containers, fuel rods, etc. If the items have separate identification marks / e.g. numbers /, or differ in enrichment, chemical composition, etc., they must form separate, separately named doses.
5. "Mass unit": The unit used in columns "Total mass," "Mass of the element" and "Weight of the isotope," i.e. it is written "kg" or "g."
6. "Isotope": Specify the type of fissile isotope the mass of which is in the column "Weight of isotope." This block is only to be completed if there is a change in inventory concerning enriched uranium.

B. Left side of the form:
7. "Name and address of consignor or device code and MBA" / i.e. the consignor or holder of the authorisation for which the inventory change is an internal change /: To be completed the exact name and address of the authorisation holder including the code assigned.
8. "Report No": All registration reports sent by the authorisation holder to the Office shall be numbered. Reports shall be numbered in the order in which they are sent to the Office, regardless of the type of messages.
9. "Data measurement method": The following cases may occur for which appropriate codes are entered in the box.

c / Special inventory changes:
In case of enrichment, exposure or mixing of different uranium categories, the nuclear material category is changed. This change must be seen as an inventory change and therefore requires the sending of an "Inventory Change Report '.
The inventory change code is again composed of two letters using the original item code as the first letter and the item code after the change as the second letter. For example, the NO code shall be used for enrichment of natural uranium.
The code belonging to the higher category according to the code for the type of inventory change shall be used under the heading "Element '. A higher category is given for uranium by the following inequalities E greater N larger D.
In the column "Weight of element ', the data in the unit belonging to the higher category shall be completed. This also applies to the heading" Mass unit'.
Other fields and fields shall be completed in the same way as for other types of inventory changes.
11. "Pol. no.": Each line in this section of the form has a numerical symbol (1, 2, 3).
12. "Total mass": The net mass of the compound, alloy, etc., which contains nuclear materials shall be indicated.
13. "% of element": The percentage of element given in the heading "Element" in the compound, alloy, etc.
14. "Element weight": an indication calculated from the total weight and percentage content of the element. If the reported nuclear material is a net element, the columns "Total weight 'and" Weight of the element' shall contain the same figure and the column "% of the element 'shall be 100. The mass data shall be given to two decimal places. The number 5 is rounded up.
15. "Weight of isotope": This column is only filled in if it is uranium with enrichment higher than natural uranium. The mass data shall be given to two decimal places. The number 5 is rounded up.
16. "Correction k": This column shall be completed in the case of the correction of the data in the previous report, allowing a link to the line message number. All fields / not only incorrect / must be completed when corrected.
17. "Note": The letter "C" shall be added to the box where the report is supplemented by additional data on a separate sheet. It shall be completed in the case of unusual inventory changes / atypical for the holder of the permit / or in the case of inventory changes LD, TU, LN, LA, GA, TW, FW, DN, ND, NE, EN, DE, ED and RF with the difference of consignor, consignee.
18. "Inventory change date": The date on which the material is sent from the authorisation holder or the operation has been carried out that led to the inventory change. In the case of nuclear production or loss, the date of the inventory change shall be the date on which the nuclear materials leave the nuclear power station or the date on which the fuel cell is removed from the core.
19. "Stamp and signature": Without the stamp of the holder of the permit and without the signature of the head of the registration of nuclear materials or his representative, the report shall be invalid and shall not be taken into account.
20. "Initial dose (s)": To be completed if part of the existing dose (s) is taken. In that case, the name of the lot dispatched must be different from that of the original lot.
"No. ': Each line in this section of the form has a numerical symbol (4, 5, 6).
"Dose name ': As in point 1. The exact name of the dose as indicated in the" Inventory of Physical Inventory' or "Document Inventory 'shall be given.
"% of element ': As point 13.
"Element weight reduction ': The weight reduction of the element shall be indicated as a result of the reported inventory change in the dose or doses referred to in the" Physical inventory inventory' or in the "Document inventory '.
"Isotope mass reduction": Analogically, according to the above and paragraph 15.
"Reduced number of items per dose ': The actual reduction in the number of items in the original dose due to the reported inventory change is reported.
"Note ': This column shall be filled in with the letter" X' only if the data to point 20 are supplemented by additional information.
C. Right page of the form / completed by the consignee /:
21. "Name and address of consignee or device code and MBA": As 7.
22. "Message No": Like 8. Each authorisation holder numbers his message independently, i.e. the message number on the left and right sides of the form is unlikely to be the same.

34. "Incorporated in benefits or benefits": To be completed when nuclear material is incorporated into existing benefits or is formed by a dose / s / with a different name.
"No. ': Each line in this section of the form has a numerical symbol (4, 5, 6).

II. Form 2 - Inventory of physical inventory
The form shall be completed in two copies, one of which shall be sent to the Office and the other retained by the holder of the authorisation for his own use. The necessary copies shall be made for the purpose of the audit work of the Office and the Agency. The form shall be completed separately for each key measuring point.
The two sections on the top right of the form are not completed, they are used for the purposes of the Office.
1. "Stamp and signature": As 19 in Form 1.
2. "Name and address of the authorisation holder or code of the authorisation holder or MBA": As 7 in Form 1.
3. "Date": The date of the physical inventory shall be completed.
4. "Key measuring point": To be completed the key measuring point code that was established when determining the key measuring points for the authorisation holder. If the Authority does not agree with the authorisation holder on the key measuring points, this means that the authorisation holder is considered to be the only key measuring point. Then this section doesn't fill in.
5. "Page No of the Parties": The page number and the total number of pages in the report shall be indicated.
6. "Report No": As 8 in Form 1.
7. "Line No": Each line in the form must be numbered (1, 2, 3... etc.). If the message is folded from multiple pages, the lines shall be numbered regardless of whether the next page continues.
8. "Dose name": As 1 in Form 1.
9. "Number of items per batch": As 4 in Form 1.
10. "Material description": As 2 in Form 1.
11. "Element": As 3 in Form 1.
12. "Total weight": As 12 in Form 1.
13. "Mass unit": As 5 in Form 1.
14. "% of element": As 13 in Form 1.
15. "Mass of element": As 14 in Form 1.
16. "Weight of isotope": As 15 in Form 1.
17. "Data measurement method": As 9 in Form 1. N code cannot be used here.
18. "Isotope": As 6 in Form 1.
19. "Note": To be completed "X" if the data in the row need to be supplemented or explained and in the case of procedure C, the letter "C" is completed.
20. "Correction K": As 16 in Form 1.
21. "Material location": The exact location of the permit holder in which the nuclear material was described at the time of physical inventory shall be specified. If the dose contains several categories of nuclear material, several consecutive rows shall be filled in. In columns "Element 'and" Mass unit', the relevant element and the relevant mass unit shall be indicated for each row. In order to indicate that the line is a continuation of the previous row, the letter "C" is entered in the column "Note."
III. Form 3 - Material Balance Report
This form shall be used to identify non-countable material and shall be completed in two copies, one of which shall be sent by the authorisation holder together with the "Inventory of Physical Inventory" form and the other shall be retained for its own use. If there are several categories of nuclear material for the permit holder, their material balance may be written in a single report.
The section on the top right of the form is not filled in, it is used for the purposes of the Office.
1. "Stamp and signature": As 19 in Form 1.
2. "Name and address of the authorisation holder or device code and MBA": As 7 in Form 1.
3. "Period from... to...": The date of the first day following the previous physical inventory and the date of the new physical inventory shall be completed.
4. "Page No of the Parties": As 4 in Form 2.
5. "Message No": As 8 in Form 1.
6. "Line No": As 8 in Form 2.
7. "Line name": The following keywords or their codes shall be used in this column in the indicated order:

9. "Element mass": Total mass for a given line. The sum of the weights from the records shall be reported.
10. "Mass unit": As 5 in Form 1.
11. "Isotope mass": The total isotope mass for a given line. The sum of the weights from the records shall be reported.
12. "Isotop": As 6 in Form 1.
13. "Note": As 19 in Form 2.
14. "Correction K": As 16 in Form 1.
IV. Form 4 - Document inventory
This form shall be processed by each authorisation holder for its own control purposes or for the purpose of the Authority's and the Agency's control activities. To be completed in the same way as Form 2.
V. Form 5 - Book inventory
This form shall be kept separately by each authorisation holder for each category of nuclear material. It shall be completed only in the original and copies of the necessary copies shall be made for the purposes of the Authority and the Agency's audit activities.
The entries in this form shall be made on the basis of the records and must be made on the date on which the change took place, or on the date on which it was found that one of the previous records was incorrect.
1. "Authorisation holder name or device code and MBA": To be completed with the name of the authorisation holder or the relevant codes.
2. "Stamp and signature": As 19 in Form 1.
3. "Element": As 3 in Form 1.
4. "Mass unit": As 5 in Form 1.
5. "Isotop": As 6 in Form 1.
6. "Page No": Each sheet of the form must be numbered.
7. "Date": The date of the change in inventory or entry shall be indicated.
8. "Type of inventory change": The keywords or codes used in Form 1 shall be used. After the physical inventory is closed, any non-countable material shall also be entered on the form. Then the modified book inventory must be equal to the physical inventory.
9. "From / to": When receiving or dispatching nuclear materials, the abbreviation or code of the authorisation holder from which the nuclear materials were accepted or sent shall be completed. In the case of an inventory change concerning only the authorisation holder, this column shall not be filled in.
10. "Dose name": As 1 in Form 1.
11. "Report No": The number of the report that the Office has been informed of the change of inventory shall be provided.
12. "Change": The weight of the element and, where applicable, the weight of the isotope by which the book inventory changed.
13. "Book inventory": The algebraic sum of the data in the previous row and the data in the column "Change" shall be reported.
14. "Note": Any additional information to the line shall be provided. It is mainly used to explain the entries concerning repairs.
In case of error in the form / wrong sum, errors in the records, etc. / it is necessary to correct the data in the line that is wrong and in case the correction affects the columns "Book inventory," all the following rows in these columns must be corrected. The number of the report which the Office has been informed of the correction shall be entered in the first corrected line.
VI. Form 6 - Export-Import Notification / EIN /
This form shall be completed with the holder of the authorisation as an accompanying document to the foreign consignment. It is filled in six copies. Two copies of the EIN shall be sent by the holder of the authorisation at the same time as the consignment, three copies shall be sent to the Office and one shall be kept for his own use. Two copies sent with material are crossed from the lower left to the upper right corner / e.g. with a red marker /.
Data of consignor, consignee and Office:
1. "Export-Import Notification No.": The holder of the authorisation shall not be filled in.
2. "Shipper": Enter the name and address of the consignor.
3. "State code" / left side of the form /: Enter the code of the sender state (for the Czech Republic it is "CZ").
4. "Receiver": The name and address of the consignee shall be indicated.
5. "State code" / right side of the form /: No authorisation holder.
