DECLARATION
Ministry of Finance
of 1 December 1999
on the method of notification of competitions, surveys and other non-consumer lottery price events
The Ministry of Finance provides pursuant to § 1 (4) of Act No. 202 / 1990 Coll., on Lottery and other similar games, as amended by Act No. 70 / 1994 Coll., Act No. 149 / 1998 Coll. and Act No. 63 / 1999 Coll.:
An operator of competitions, poll or other events for prizes in which the sum of non-monetary winnings for one calendar year does not exceed CZK 200 000 and the value of the individual winnings does not exceed CZK 20 000 (hereinafter referred to as "the operator") in writing to the relevant financial authority (1) not later than 15 days before the date of the opening of the notification referred to in § 2.
(1) The notification shall contain the following information:
(a) for legal persons, the trade name, identification number and registered office, the name, surname, birth number, otherwise the date of birth, place of permanent residence, business name, identification number and place of business, if different from the place of permanent residence,
(b) the name, surname, birth number, date of birth and place of permanent residence of the person or persons who are the statutory authority or its members, indicating the extent of the authorisation to act and sign on behalf of the legal person;
(c) name, brief description of the competition, surveys and other price events (e.g. advertising for a particular product, product or service);
(d) the duration of the competition, the survey or other price events, i.e. the exact date of its opening and closing. End date means the date of the last composition or otherwise random selection of the winners of the competition, surveys and other price events,
(e) a summary of the non-monetary wins in the monetary terms to which the operator undertakes to provide and the value of the individual wins. The cash expression of non-cash wins shall be made in accordance with a special regulation, 2)
(f) the place, manner and time limits for the transmission of wins.
(2) The notification shall also be subject to any changes to the data referred to in paragraph 1 within 15 days of the date of the change.
(3) A separate notification shall be submitted for each individual contest, poll or other price action.
(4) Where the opening and closing of a competition, a query or other price action is spread over several calendar years, the operator shall indicate in the notification the sum of the non-cash wins and the value of each win in the breakdown by calendar year.
This decree shall take effect on 1 January 2000.
