DECLARATION
of 16 December 2014
on the labelling and passports of pet dogs, cats and ferrets in their non-commercial movements
The Ministry of Agriculture provides pursuant to § 78 of Act No. 166 / 1999 Coll., on Veterinary Care and on the amendment of certain related laws (Veterinary Act), as amended by Act No. 131 / 2003 Coll., Act No. 316 / 2004 Coll., Act No. 48 / 2006 Coll., Act No. 182 / 2008 Coll., Act No. 227 / 2009 Coll., Act No. 308 / 2011 Coll., Act No. 359 / 2012 Coll., Act No. 279 / 2013 Coll., Act No. 139 / 2014 Coll., and Act No. 264 / 2014 Coll., for the implementation of Section 4 (7) of the Veterinary Act:
Subject matter
This decree regulates the application of directly applicable European Union1)
(a) the marking of pet dogs, cats and ferrets (hereinafter referred to as pet animals) in their non-commercial movements;
(b) the method of production, issue and numbering of passports,
(c) the manner and time limits for the transmission of data to the passport register, the manner in which it is kept and the content thereof;
(d) requirements for recording data on the passport; and
(e) model passport.
Identification of pet by transponder
The designation of the pet animal transpondérem2) shall be carried out by a veterinarian or a person competent under Section 59 of the Veterinary Act.
Method of production, issue and numbering of passports
(1) Passport manufacturer
(a) a transparent adhesive film in such a way that Section III of the passport can be sealed,
(b) 10 pieces of transparent adhesive sheets so that they can seal the particulars in Section V of the passport if they are in the form of a sticker, provided that the label is not destroyed when the label is removed; and
(c) 16 pieces of transparent adhesive sheets, so that they can seal the particulars in Sections VII, VIII and IX of the passport if they are in the form of a sticker, provided that the label is not destroyed when removed.
(2) The registration number contains:
(a) in the first two places, the letters "CZ" and
(b) in the following locations, the number of the series allocated by the Central Veterinary Administration in accordance with Article 21 (3) of the European Union Code on animal health conditions governing the non-commercial movement of pet animals.
Method and time limits for the transmission of data to passport records, the manner in which such records are kept and the content thereof
(1) The Chamber shall keep records in a manner that allows remote access.
(2) The records shall contain:
(a) the particulars referred to in Article 23 (1) of the European Union Code on animal health conditions governing the non-commercial movement of pet animals,
(b) the name and, where applicable, the name and surname and address of the place of residence of the owner of the pet animal and, where applicable, of the owner of the pet animal abroad;
(c) the means of identification and the number of the transponder or tattoo of the pet animal;
(d) the passport registration number; and
(e) particulars of the pet animal:
1. species,
2nd breed,
3. date of birth,
4.
color 5.
(3) Data on passports delivered and issued shall be transmitted via an electronic application accessible via the Chamber website.
(4) The passport manufacturer shall transmit the data on the passports delivered at the latest on the date of delivery to the passport distributor or approved veterinarian, if the passport manufacturer is simultaneously a passport distributor.
(5) The passport distributor shall transmit the data on the passports delivered at the latest on the date of delivery to the approved veterinarian.
(6) The authorised veterinarian shall transmit the passport data referred to in paragraph 2 (b) to (e) within 3 working days of issue of the passport.
Passport Model
Model 3) is annexed to this decree. The model of the passport set out in the Annex shall be without prejudice to the additional requirements for its issue set out in Annex III to Commission Regulation (EU) No 577 / 2013.
Repeal
The following shall be deleted:
1. Decree No. 610 / 2004 Coll., on the labelling and passports of dogs, cats and ferrets in their non-commercial movements and amending Decree No. 296 / 2003 Coll., on animal health and protection, on the movement and transport of animals and on the authorisation and competence to perform certain professional veterinary activities.
2. Point 2 § 42 of Decree No. 342 / 2012 Coll., on animal health and protection, on the movement and transport of animals and on the authorisation and competence of certain professional veterinary activities.
Efficacy
This Decision shall enter into force on 29 December 2014.

Passport Model
1) Regulation (EU) No 576 / 2013 of the European Parliament and of the Council of 12 June 2013 on the non-commercial movement of pet animals and repealing Regulation (EC) No 998 / 2003. Commission Implementing Regulation (EU) No 577 / 2013 of 28 June 2013 on model identification documents for the non-commercial movement of dogs, cats and ferrets, drawing up a list of territories and third countries and the format, layout and language requirements for declarations confirming compliance with certain conditions laid down in Regulation (EU) No 576 / 2013 of the European Parliament and of the Council.
2) Article 17 (1) of Regulation (EU) No 576 / 2013 of the European Parliament and of the Council.
