DECLARATION
of 13 June 2006
amending Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Names and Surnames, and amending certain related acts, as amended
The Ministry of the Interior provides pursuant to Section 96 of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related laws, as amended by Act No. 165 / 2004 Coll.:
Amendment of Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Names and Surnames and on the Amendment of Certain Related Acts, as amended
Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Names and Surnames, and on the Amendment of Certain Related Acts, as amended by Decree No. 230 / 2001 Coll., Decree No. 499 / 2001 Coll., Decree No. 566 / 2002 Coll., Decree No. 660 / 2004 Coll. and Decree No. 539 / 2005 Coll., is amended as follows:
1. in Article 2, the following point 12 is inserted after point 11:
'12. The death certificate, indicating the data of the living partner, has resulted in death for the duration of the partnership '.
Points 12 to 15 shall be renumbered 13 to 16.
2. In Annex No 2, in Model 7, the words "Living Husband 'are replaced by the words" Living Husband / Partner'.
3. In Annex 2, the following Model 12 is inserted after Model 11:
"MODEL 12
Death certificate, indicating the living partner's data, if the death occurred for the duration of the partnership
"
The models from 12 to 15 are renumbered 13 to 16.
The matrix form "List of the Matrix Death Book 'in the form of existing legislation may be used until 31 December 2020 at the latest, provided that the amendment referred to in Section 23 (2) is indicated.
EFFECTIVE
This Decree shall take effect on 1 July 2006.
