The Ministry of the Interior provides pursuant to § 26 (a), (c) and (d) of Act No. 328 / 1999 Coll., on Civil Documents, as amended by Act No. 559 / 2004 Coll., and pursuant to § 25 (a) of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (Act on the registration of residents):

In Decree No. 296 / 2004 Coll., implementing the Act on the Registration of Residents and amending Decree No. 177 / 2000 Coll., implementing the Act on the Registration of Residents, the Act on Civil Documents and the Act on Travel Documents, as amended, as amended, as amended by Decree No. 661 / 2004 Coll., Annex 3 reads:

Transitional provision

The form of the request for data from the information system issued in accordance with the model of this form provided for by Decree No. 296 / 2004 Coll., as effective until the date of entry into force of this Order, may be used no later than 31 December 2010.

This Decree shall take effect on 1 July 2006.
