DECLARATION
of 29 August 2012
on the particulars of the Charter concerning the examination of the deceased, the manner in which he is to be completed and the transfer to the places of destination (Order on the Charter on the examination of the deceased)
The Ministry of Health provides pursuant to Section 120 of Act No. 372 / 2011 Coll., on Health Services and the Conditions for their Provision (Health Services Act), as amended by Act No. 167 / 2012 Coll., for the implementation of Section 92 of the Health Services Act:
(1) The certificate of inspection of the deceased (hereinafter referred to as "the sheet") shall be completed in four copies.
(2) The particulars of the Charter and the instructions for its completion are set out in the Annex to this Order.
(3) For the deceased, a stillborn child is also meant for the completion of the List.
(1) If no obligation to perform an autopsy or not to perform an autopsy is provided for by the Health Services Act, 1 copy of the certificate
(a) transmit:
1. to the Matrix Office under the Matrix Act (1),
2. funeral service operators providing transport of the deceased to ensure burial; and
3. The Institute of Health Information and Statistics of the Czech Republic, no later than 30 days from the date of death or the date of the body of the deceased,
(b) establish in the medical file a provider whose doctor has examined the deceased's body.
(2) If there is an obligation to perform an autopsy or to perform an autopsy, all copies of the List shall be transmitted to the provider in whose medical establishment an autopsy is to be carried out.
(3) In the case of a decision not to perform an autopsy, paragraph 1 (a) shall apply mutatis mutandis; 1 of the copy of the List is incorporated into the medical file of the deceased health service provider in whose medical establishment an autopsy was to be carried out.
(4) Upon completion of the autopsy and completion of the data sheet, the relevant copies of the certificate shall be transmitted to the places of destination referred to in paragraph 1 (a) and 1 of the copy of the certificate shall be entered in the health documentation kept by the provider at whose medical establishment the autopsy has been carried out.
The sheet shall be transmitted in paper form.
Corrections to the data in the List shall be made where, on the basis of diagnostic examinations carried out or investigations carried out by law enforcement authorities, additional facts are not identified in the parts of the Charter transmitted. The health service provider who finds such a change shall make a correction by completing the new List, which shall be transmitted again to the destination referred to in Section 2. At the same time, they will indicate on the List that it is a repair. The same procedure is followed when it comes to correcting other errors relevant to the correct completion of the List. The newly completed List replaces the original.
This Decree shall take effect on 20 September 2012.
