DECLARATION

of 21 January 2016

laying down models of diplomatic and service passports and travel cards and model forms of applications for them and application forms for amendments or additions to the travel licence data

The Ministry of Foreign Affairs provides for the Act No. 329 / 1999 Coll., as amended by Act No. 559 / 2004 Coll., Act No. 136 / 2006 Coll., Act No. 106 / 2007 Coll., Act No. 140 / 2008 Coll., Act No. 197 / 2010 Coll., Act No. 424 / 2010 Coll. and Act No. 318 / 2015 Coll.:

§ 1

Model diplomatic and service passports

(1) The model of the diplomatic passport, which contains machine-readable data and a biometric data carrier, is set out in Annex 1 to this decree.

(2) The model service passport, which contains machine-readable data and a data carrier with biometric data, is set out in Annex 2 to this Decree.

(3) The machine-readable data are recorded in the designated zone at the bottom of the diplomatic and service passports listed in Annexes 1 and 2 to this Regulation.

(4) The biometric data carrier shall be located on the other side of the diplomatic and service passports listed in Annexes 1 and 2 to this Decree.

(5) Diplomatic passport and service passport are travel documents of the type of book and contain 34 pages.

(6) The unabridged form of the name and, where appropriate, the names and surnames shall be entered in the official records on page 3 of the diplomatic and service passports listed in Annexes 1 and 2 thereto.

(7) On pages 6 and 7 of the diplomatic and service passports listed in Annexes 1 and 2 to this Regulation, the register (1) lists the names of the relevant entries entered in these passports in the German, Italian, Spanish, Portuguese, Dutch, Swedish, Danish, Finnish, Greek, Irish, Slovak, Slovenian, Polish, Hungarian, Maltese, Estonian, Latvian, Russian, Chinese and Arabic languages.

§ 2

Model travel licence

(1) The model of the travel document is set out in Annex 3 to this Decision.

(2) The travel document is a travel document of a type of book and contains 8 pages.

(3) The unabridged form of the name or, where applicable, the names and surnames shall be entered in the official records on page 8 of the travel document.

§ 3

Models of application forms

(1) The model of the application form for the issue of a diplomatic passport containing machine-readable data and a biometric data carrier is set out in Annex 4 to this Decree.

(2) The model of the application form for a service passport containing machine-readable data and a biometric data carrier is set out in Annex 5 to this Regulation.

(3) The model of the application form for a travel document is set out in Annex 6 to this Decision.

(4) The model of the application form for changes to the travel licence data is set out in Annex 7 to this decree.

§ 4

Repeal

Decree No 79 / 2012 Coll., on the establishment of models of diplomatic and service passports and forms of applications for extradition shall be repealed.

§ 5

Efficacy

This Decision shall enter into force on 1 February 2016.
