DECLARATION
of 15 September 2010
to implement certain provisions of Act No. 296 / 2009 Coll., on census, houses and flats in 2011
Pursuant to Section 30 of Act No. 296 / 2009 Coll., on the census of people, houses and flats in 2011 (hereinafter referred to as "the Act"), the Czech Statistical Office provides for the implementation of Sections 7 (1) (b), 13 (5) and 18 (8) of the Act:
Models of addition forms
(1) The model of the census form "The census sheet of a person" in paper form is set out in Annex 1 to this decree.
(2) The model of the census form "Housing sheet" is given in paper form in Annex 2 to this Decree.
(3) The model of the census form "Home certificate" in paper form is set out in Annex 3 to this Decree.
(4) The models of the addition forms in electronic form (hereinafter referred to as "electronic forms") are in substance identical and graphically similar to those of the addition forms in paper form (hereinafter referred to as "paper forms").
Registration code of the census form
(1) The paper form shall indicate its registration code, which shall be used to monitor the movement of the form during its distribution and collection up to the entry of the data processing form. It shall also contain a confirmation code used to confirm the data on access to the electronic form of the form. The registration code and the confirmation code are the access password.
(2) The accounting code of the census form is an alphabetical-digit figure. On the paper form it is expressed by a barcode with the underprint value of this data. The confirmation code is a six-digit alphabetical-digit figure.
(3) The accounting code of the census form and the validation code do not contain other personal data (1).
Electronic forms
(1) Electronic forms take the form of a PDF document.
(2) The electronic forms are obtained by obliged persons under Section 7 of the Act on the census website using the registration and confirmation code.
(3) After the online sending of the data from the form, a new page of this form is created, containing confirmation of the successful receipt of the data by the Czech Statistical Office (hereinafter "the Office"). This confirmation includes a time stamp and a unique confirmation code. Modifications cannot be made on the form after dispatch. When sending the form using the data box, the delivery confirmation is created by standard data box tools.
Model of the census commissioner's card
(1) The card shall take the form of a plastic card with dimensions 86 x 54 mm and shall bear the following particulars:
(a) brand and logotype of the 2011 census, houses and apartments,
(b) the title, name and surname of the adding Commissioner,
(c) the definition of the territorial scope of the census commissioner, that is the name of the district and the municipality and the code of the census or circuits in which the census commissioner is authorised to carry out the census,
(d) the card number of the adding commissioner,
(e) the period of validity of the licence;
(f) a photograph of the adding Commissioner,
(g) a protective element which makes it difficult to forge a licence.
(2) The staff of the Office, the ministries referred to in § 9 (1) to (4) of the Act and the staff of the service supplier carrying out the inspection activity in relation to other census Commissioners shall bear in the document adding the Commissioner's "Territorial scope" the entry "The entire territory of the Czech Republic" or the name of the region in whose territory they are authorised to carry out the inspection activity.
(3) The model of the Compilation Commissioner's Document is set out in Annex 4 to this Order.
Efficacy
This Decree shall take effect on 1 December 2010.
