The Ministry of Foreign Affairs provides, pursuant to § 5 (3) (b) and § 20 (4) of Act No. 329 / 1999 Coll., on travel documents and amending Act No. 283 / 1991 Coll., on the Police of the Czech Republic, as amended, (Act on travel documents):
Model travel documents
(1) The model of the diplomatic passport without machine-readable data is set out in Annex 1.
(2) The model of the service passport without machine-readable data is set out in Annex 2.
(3) The model of the diplomatic passport with machine-readable data is set out in Annex 3.
(4) The model of the service passport with machine-readable data is set out in Annex 4.
Models of application forms
(1) The model of the application form for a diplomatic passport or service passport is set out in Annex 5.
(2) The model of the application form for changes to the data and entry of the child in the diplomatic or service passport is set out in Annex 6.
Efficacy
This decree shall take effect on the day of its publication.
