DECLARATION
of 31 October 2018
laying down the formalities and models for evidence of specialisation education and certain other evidence of training of doctors, dentists and pharmacists
The Ministry of Health shall determine, in accordance with § 37 (1) (r) and (s) of Act No. 95 / 2004 Coll., the conditions for obtaining and recognising professional competence and specialised competence for the performance of the medical profession of physician, dental practitioner and pharmacist, as amended by Act No. 67 / 2017 Coll., hereinafter referred to as "the Act ':
Subject matter
This decree regulates the requirements and models
(a) a certificate of completion of the basic strain;
(b) a diploma on specialised competence;
(c) a diploma on specific specialised competence; and
(d) licences for the completion of a functional course;
(hereinafter referred to as education documents).
Forms of educational documents
(1) The general requirements of the educational documents are:
(a) the designation of the Ministry of Health, together with a small state emblem of the Czech Republic,
(b) the particulars of the person to whom the document is issued, namely the academic title, the name and, where applicable, the names and surnames, the date of birth, the place of birth;
(c) the number of the education document in the complete numerical series;
(d) the name of the educational document;
(e) the date of issue of the educational document; and
(f) the name and, where appropriate, the name, surname, academic title and signature of the person acting for the Ministry of Health and the name and, where appropriate, surname, academic title and signature of the person acting for the entrusted organisation.
(2) The special requirements of the basic strain certificate are:
(a) the date of completion of the post-basic school examination or the date of completion of the basic school education if the person to whom the document is issued has not been required to pass the basic school examination after completion of the primary school course;
(b) the designation of the baseline strain; and
(c) the stamp (1) of the Ministry of Health and the name and / or names, surnames, academic title and signature of the Chairman of the Examination Board.
(3) The specific requirements of the diploma on specialised competence are:
(a) the date of completion of the test;
(b) designation of specialisation;
(c) the designation of the health profession;
(d) the stamp (1) of the Ministry of Health and the name and / or names, surnames, academic title and signature of the Chairman of the Attestation Commission; and
(e) for specialisation fields listed in Annex V. to Directive 2005 / 36 / EC of the European Parliament and of the Council of 6 July 2005 on the recognition of professional qualifications, the sentence "Specialised competence is in accordance with Article 25 of Directive 2005 / 36 / EC of the European Parliament and of the Council of 6 July 2005 on the recognition of professional qualifications and the document issued is evidence of the training referred to in Annex V (5.1.2) and (5.1.3) to this Directive."
(4) The special requirements of the diploma on specific specialised competence are:
(a) the date of completion of the final test of the superstructure;
(b) the designation of the superstructure;
(c) the indication of the health profession; and
(d) the stamp (1) of the Ministry of Health and the name and / or name, surname, academic title and signature of the Chairman of the Examination Board.
(5) The special terms of the licence for the completion of the functional course are:
(a) the date of completion of the final test of the functional course;
(b) the designation of the functional course; and
(c) the stamp (1) of the Ministry of Health and the name and / or names, surnames, academic title and signature of the Chairman of the Examination Board.
Further particulars of the educational documents
(1) Education documents shall be drawn up on a sheet of 210 x 297 mm, weighing 100 g / m2 with 50% cotton content. The certificate form paper is without its own fluorescence in UV light and is equipped with
(a) by an intermediate positive-negative watermark with the motifs of the leaves of lime;
(b) horizontal guilloche motif,
(c) blue fluorescent safety fibres in UV light; and
d) security fibres optically invisible with green fluorescence in UV light.
(2) Evidence of education is printed using:
a) offset printing with 4 colours on the face with iris transitions in 2 colours,
b) Line rotogravure in 1 black on the face for a small national emblem of the Czech Republic,
(c) printing in black with green fluorescent in UV light for a six-digit serial number;
(d) different colours for printing the title of the educational document, for the title of the educational document referred to in § 2 (2) in blue, for the title of the educational document referred to in § 2 (3) in red, for the title of the educational document referred to in § 2 (4) in orange and for the title of the educational document referred to in § 2 (5) in green,
(e) guilloche motifs;
(f) the application of pastel shades which make it difficult to obtain copies on laser copying machines; and
(g) the protective element printed by the liner rotogravure technique.
(3) The forms of the educational documents contain a special protection element classified in the level of secrecy reserved.
Models of educational documents
(1) The model of the certificate of completion of the basic strain is set out in Annex 1 to this Decree.
(2) The model of the diploma on specialised competence is set out in Annex 2 to this decree.
(3) For the specialisation fields listed in Annex V. to Directive 2005 / 36 / EC of the European Parliament and of the Council of 6 July 2005 on the recognition of professional qualifications in the form referred to in Section 2 (3) (e), the model of the diploma on specialised competence is set out in Annex 3 to this Decree.
(4) The model of the diploma on specific specialised competence is set out in Annex 4 to this decree.
(5) The model of the licence to complete the functional course is set out in Annex 5 to this order.
Efficacy
This decree shall take effect on the first day of the seventh month following its publication.
