DECLARATION
of 19 May 2006
laying down the model of the European Arrest Warrant and the model of the certificate of issue of the freezing order
The Ministry of Justice provides, pursuant to § 405 (5) and § 460b (2) of Act No. 141 / 1961 Coll., on criminal proceedings, as amended by Act No. 253 / 2006 Coll.:
This decree (1) implements the relevant provisions of the European Union2) and provides for
(a) a model of the European arrest warrant;
(b) a specimen of the certificate of issue of the freezing order.
The model of the European Arrest Warrant is set out in Annex 1 to this Order.
The model of the certificate of issue of the freezing order is set out in Annex 2 to this order.
This Decree shall take effect on 1 July 2006.

Certificate of issue of the freezing order
1) It is issued on the basis and within the limits of the law in which the relevant European Union framework decisions have already been reflected.
2) Annex to Council Framework Decision 2002 / 584 / JHA of 13 June 2002 on the European arrest warrant and surrender procedures between Member States. Annex to Council Framework Decision 2003 / 577 / JHA of 22 July 2003 on the execution of orders for property or evidence in the European Union.
