laying down the model of the staff member's card in the Territorial Financial Authority
The Ministry of Finance provides pursuant to Section 13a of Act No. 531 / 1990 Coll., on Territorial Financial Bodies, as amended by Act No. 311 / 1999 Coll., Act No. 444 / 2005 Coll. and Act No. 199 / 2010 Coll.:
The model of the staff member's service card in the Territorial Financial Authority is set out in the Annex to this Order.
This Decree shall take effect on 1 January 2011.

Model of service of staff in the Territorial Financial Authority
1. Description
(a) On the face of the service card, the first line shall bear the name of the territorial financial authority in which the staff member carries out the operations in accordance with Act No. 531 / 1990 Coll., on Territorial Financial Authorities, as amended. The second line shows the personal number of the employee, the third line contains his title, name, or surname. The fourth line shows the Directorate-General for Finance as the authority issuing the licence. The fifth line shows the series and the licence number. The sixth line shows the date of issue of the licence. On the left is a photograph of the employee. The top right is a big state emblem. On the bottom right is a place for electronically printed handwritten signature by the Director-General of the General Financial Directorate. In the lower part, the words "The holder of this service card is an employee in the Territorial Financial Authority and is entitled to carry out acts in accordance with Act No. 531 / 1990 Coll., on Territorial Financial Authorities, as amended. '
(b) The employee card in the Territorial Financial Authority shall be 99 x 69 mm and shall be stored in a plastic package of 105 x 75 mm. The security features are the price paper with watermark, the price grid with a combination of colours making it difficult to get copies on colour laser printers and numbering with the order number in UV. The requirements for the technical execution of the photograph shall comply with the technical requirements for photographs for the issue of the identity card.
(c) The reverse side of the service card shall be free of printing.
