DECLARATION
of 1 July 2025
amending Decree No 373 / 2016 Coll., on the transmission of data to the National Health Information System, as amended
The Ministry of Health provides, pursuant to Section 120 of Act No. 372 / 2011 Coll., on Health Services and Conditions of Provision (Act on Health Services), as amended by Act No. 147 / 2016 Coll., Act No. 326 / 2021 Coll. and Act No. 236 / 2025 Coll., for the implementation of Section 78 of the Act on Health Services:
Čl. I
Decree No. 373 / 2016 Coll., on the transmission of data to the National Health Information System, as amended by Decree No. 115 / 2019 Coll., Decree No. 389 / 2021 Coll. and Decree No. 388 / 2024 Coll., is amended as follows:
1. In Article 3, the following paragraph 3 is added:
"(3) The data relating to the transmission of the results of the laboratory tests provided for in Section 77a (4) (a) of the Health Services Act are set out in Annex 3 to this Decree."
2. In Annex 1, point 18.2.4, "24 hours' is replaced by" 14 days'.
3. In Annex 1, point 18.3 is added:
"18.3. Data on the results of laboratory tests carried out:
The data relating to the transmission of the results of the laboratory tests carried out pursuant to § 77a (4) (a) of the Health Services Act are set out in Annex 3 to this Decree. '
4. In Annex 1, point 19 is added:
'19. National register of preventive and screening tests
19.1 Heading of data-transmission providers:
Any provider who has been involved in preventive or screening programs published by the Ministry in the Ministry of Health Bulletin.
19.2 Periodicity and transmission deadlines:
The data shall be transmitted within one month of the provision of the health service. ';
5. The following Annex 3 is added, including the title:

"Annex No 3
Data on the results of laboratory tests carried out pursuant to § 77a (4) (a) of the Health Services Act
1. Results of expert laboratory examinations to be transmitted pursuant to § 77a (4) (a) of the Health Services Act.
(a) For the assessment of cardiometabolic risk, the value for
1. glycated haemoglobin (HbA1c),
2. plasma glucose (recorded after fasting),
3. total cholesterol,
4. HDL cholesterol,
5. LDL cholesterol (test or calculation),
6. triglycerides,
7. lipoprotein Lp (a),
8. NT- proBNP,
9. ureu,
10. tyreotropin (TSH),
11. T4 free,
12. For protein total,
13. albumin in urine,
14. creatinine in urine,
15. Serum creatinine,
16. anti-GAD,
17.C-peptide,
18. ACR (Album / creatine ratio),
19. Estimation of glomerular filtration (eGFR) (calculated from CDK-EPI creatine concentration),
20.25-hydoxyvitamin D (calcium) total.
b) For the assessment of the risk of oncological diseases, the observed value for
1. occult bleeding in the stool (TOKS),
2. high risk HPV,
3. prostatic specific antigen (PSA) total,
4. free prostatic specific PSA antigen,
5. p2PSA.
2. In relation to the results of the laboratory tests provided for in Section 77a (4) (a) of the Health Services Act, information on whether the examination is carried out
(a) the primary result of requested laboratory tests in connection with the clinical indication; or
(b) secondary resulting from the primary laboratory examination. "
Čl. II
Efficacy
This Decree shall take effect on the first day of the second calendar month following its publication, with the exception of Article I (4), which shall take effect on 1 January 2026.
