laying down a model for the written delegation of a control officer to carry out research control on human embryonic stem cells and related activities
The Ministry of Education, Youth and Sports (hereinafter referred to as "the Ministry ') provides, pursuant to Section 14 (2) of Act No. 227 / 2006 Coll., on research on human embryonic stem cells and related activities and on the amendment of certain related laws:
Model of written mandate
The model of the written mandate of a control officer to carry out research control on human embryonic stem cells and related activities is set out in the Annex to this decree.
Efficacy
This decree takes effect on the date of the entry into force of Act No. 227 / 2006 Coll., on research on human embryonic stem cells and related activities and on the modification of certain related laws.

Model of the written delegation of the control officer carrying out research control on human embryonic stem cells and related activities pursuant to Section 14 of Act No. 227 / 2006 Coll.
Front of the written mandate
The written mandate is printed on a white paper card with dimensions of 68 mm x 99 mm (international ID 2 format), covered with a transparent laminating sheet.
The text on the front is made in black. The upper part of the written mandate includes the designation "Ministry of Education, Youth and Sports." A small national character is printed in colour on the top left, and a written mandate number on the right. In the left part under the number of the written mandate, the field for the colour photograph of the inspector is left with 30 mm x 35 mm printed round stamp of the Ministry. In the right part of the mandate, the name "Mandatory 'is marked in bold and contains the text" Controller of Research in Human Embryo Stem Cells and Related Activities under Section 14 of Act No. 227 / 2006 Coll.'. The following text shall be entered under this text in capital letters: "NAME, FOREIGN. NAME AND RECEIVATION (ACADEMIC TITULE, RESPECT OF SCIENTIFIC STANDARDS):" and leave room for such data to be provided. The following information shows the text in capital letters as follows: "CIVIL TRADE NUMBER: 'and the space for this information is left.
Back of written mandate
The back of the written mandate shall contain the text with the authority to carry out the control activity as follows: "I hereby authorise the holder of this mandate to carry out the control of research on human embryonic stem cells and related activities." Under this text in the right-hand section is the signature of the Minister, who will be reprinted with the Ministry's round stamp. The lower part of the mandate contains a text indicating the place and date of issue as follows: "In Prague on: '. The following text is given below:" In case of a finding, give this mandate to the Police of the Czech Republic or to the Ministry of Education, Youth and Sports. "The text on the tax side is made in black.
