DECLARATION

amending and supplementing the Decree of the Ministry of Health of the Czech Socialist Republic No. 91 / 1984 Coll., on measures against communicable diseases, as amended by Decree No. 204 / 1988 Coll., and amending the Directive of the Ministry of Health No. 30 / 1968 Bulletin of the Ministry of Health, on measures against sexual diseases (registered in the amount of 51 / 1968 Coll.)
The Ministry of Health provides pursuant to § 70 (1) (c) of Act No. 20 / 1966 Coll., on the Care of People, as amended by the Act of the Czech National Council No. 548 / 1991 Coll.:
The Decree of the Ministry of Health of the Czech Socialist Republic No. 91 / 1984 Coll., on measures against communicable diseases, as amended by the Decree of the Ministry of Health and Social Affairs of the Czech Socialist Republic No. 204 / 1988 Coll., is amended as follows:
1. in Paragraph 34 (1), footnote 10a shall read:
"10a) Form SEVT No 14 79 00" Reporting of communicable disease. "Form NZIS-011 1" Compulsory reporting of tuberculosis and other mycobacteriosis'. Form NZIS-012 1 "Control report on tuberculosis and other mycobacteriosis'. Form NZIS-013 1" Data change report for persons in RT '. Form NZIS-017 0 "Report of VD."'
2. In Annex 3, points 8 and 12 are deleted. Points 9 to 11 shall be renumbered 8 to 10.
The Annex to the Directives of the Ministry of Health No 30 / 1968 The Bulletin of the Ministry of Health, on measures against sexual diseases registered in the amount of 51 / 1968 Coll.
This decree shall take effect on the day of its publication.
