DECLARATION
of 30 May 2005
on certain documents relating to education
The Ministry of Education, Youth and Sports provides according to § 28 paragraphs 7 and 8 of Act No. 561 / 2004 Coll., on Pre-school, Basic, Medium, Higher Vocational and Other Education (Education Act):
Content and form of valid forms of certificate, certificate and diploma of discharge
(1) The content and form of the valid forms of the certificate, of the forms of education and of the diplomas of discharge are laid down in paragraphs 4 to 7 and in the annexes to this decree, which provide for models of the forms.
(2) Models of valid certificate forms for assessing the results of the pupil's education
(a) the primary school is listed in Annex 1 to this order,
(b) the special school is listed in Annex 2 to this decree;
(c) in the secondary school and in the Conservatory they are listed in Annex 3 to this Decree,
(d) the basic art school is listed in Annex 4 to this decree;
(e) in the event of successful completion of examinations or repair tests in the course of basic education, they are listed in Annex 5 to this Decree.
(3) Models of valid forms
(a) the certificate of final examination and the teaching certificate are set out in Annex 6 to this order;
(b) a certificate of discharge and a diploma of discharge at a higher vocational school are set out in Annex 7 to this decree;
(c) the certificate of discharge and the diploma of discharge in the Conservatory, the model of the certificate of graduate examination of pupils who were due to complete secondary education through the graduate examination before 1 September 2010, are set out in Annex 8 to this decree,
(d) the national language examination certificate is set out in Annex 9 to this Decree;
(e) a certificate of completion of secondary education by a graduate examination after 1 September 2010 is given in Annex 8a to this Decree.
(4) The forms of the certificate, the certificate of education and the certificate of discharge shall be accompanied by a grey underprint with the motif of a small national character and of lipid leaves on each side of the form. The forms referred to in paragraph 3 shall also bear a watermark with the motifs of the lipid branches.
(5) The forms referred to in paragraph 3 shall bear a serial number and at least a six-digit number of the form. The same shall apply to the forms referred to in paragraph 2 if they are to be used for the issue of a certificate, which is evidence of the degree of education achieved (1).
(6) The forms referred to in paragraph 2 shall be drawn up on a sheet or, where appropriate, a double sheet of 210 x 297 mm format with a tolerance of 3 mm. The forms referred to in paragraph 3 shall be drawn up on a sheet of 210 x 280 mm format with a tolerance of 3 mm.
(7) The location of the school department identifier (2) is given on the corresponding forms by the abbreviation "IZO." The place for listing of the department identifier of a legal person carrying out the activity of the school (8) shall be given by the abbreviation "RED IZO."
Content and form of extracts from the certificate
The extract of the certificate shall contain the particulars resulting from the model form of the certificate set out in the Annexes to this Order. The extract of the certificate shall be stamped by the legal person carrying out the school's activities (hereinafter referred to as the "stamp ') and by the name, surname and signature of the person issuing the extract.
Copy and copy of the certificate, certificate and diploma of discharge
(1) A copy of the certificate, the certificate and the diploma of discharge (hereinafter referred to as the copy) shall be issued by the school (3) at the request of the person to whom the original was issued or to its legal representative. The copy shall be issued as a copy of the original submitted and shall bear the stamp, name, surname and signature of the person issuing the copy and the date of issue of the copy. Where the original consists of several separate sheets, these particulars shall be indicated on each sheet. The issue of the copy shall be indicated in the relevant school documentation (4).
(2) The same copy of the certificate, certificate and diploma of discharge (hereinafter referred to as the "copy") shall be issued by the school at the request of the person to whom the original was issued or to its legal representative. The same copy shall be drawn up on the appropriate form in force at the time of issue of the original or, if this form is not available, on another appropriate form. The issue of the copy shall be indicated in the relevant school documentation (4).
(3) Where the signatures of the persons who have signed the original cannot be affixed, the original shall appear on the copy of the name and surname of the persons who have "v. r. '. Where the stamp used on the original cannot be affixed, the abbreviation" L. S. "shall appear on the copy instead of that imprint. The endorsement" This copy conforms to the original' shall be affixed to the copy. The following particulars shall appear on the document: name, surname, signature of the principal of the school which issued the copy, stamp and date of copy. Where a copy is made out on several separate sheets, each copy shall be accompanied by a document and particulars.
(4) Where a school which has issued an original is no longer in existence, a copy shall be issued in accordance with the procedure laid down in paragraphs 2 and 3 by the school which imposes its documentation, or by another school providing education in the same or similar field of education, provided that the applicant submits to him copies of the documents on the basis of which the copy may be issued, drawn up in accordance with the Charter and File Service Act and amending certain Laws (5) or certified under the Verification Act (7).
(5) If the school documentation to which the application for a copy relates has been destroyed, the school shall issue a certificate to the applicant.
(6) Where a copy is issued because of a change in the name, surname or birth number, the applicant's name and surname and the birth number shall be indicated on the copy as at the date of issue of the copy. The application for the issue of a copy shall be accompanied by the original of the certificate, the certificate or the diploma of discharge, and by evidence of the change of name, surname and birth number. The change of name or surname shall be evidenced by a decision of the competent administrative authority authorising the change of name and surname (5a) or, where appropriate, an officially certified copy thereof. A change in the birth number shall be evidenced in accordance with other legislation5b).
Common provision
For the purposes of this decree, the serial number and the number in the class statement shall be the serial number under which the pupil is kept in the pupil register (6).
Transitional provisions
(1) For the period up to the end of the school year 2004 / 2005, there are models of valid certificate forms containing the evaluation of student education results.
(a) only at the first stage of the primary school in Annex 10 to this order;
(b) only at the second stage of the primary school in Annex 11 to this Decree;
(c) in the first and second stages of the primary school in Annex 12 to this Decree,
(d) in a secondary school, with the exception of the forms of certificates referred to in point (e), in Annex 13 to this Order,
(e) in a secondary school containing a clause on obtaining a degree of basic education in Annex 14 to this Decree;
(f) in the Conservatory in Annex 15 to this Decree,
(g) at the primary art school in Annex 16 to this Decree,
(h) in the event of termination of the course for basic education and completion of the training provided by the primary school, special in Annex 17 to this Decree.
(2) For the period up to the end of the 2004 / 05 school year, models of valid forms are given
(a) the teaching certificate and the final examination certificate in Annex 18 to this Decree;
(b) a certificate of discharge and a diploma of discharge at a higher vocational school in Annex 20 to this Order;
(c) a certificate of discharge in the Conservatory and Diploma of discharge in the Conservatory in Annex 8 thereto,
(d) the certificate of graduation and the certificate of graduation in the Conservatory in Annex 19 to this Decree;
(e) the Certificate of State Language Examination in Annex 9 to this Decree.
(3) The forms referred to in paragraph 1 shall be accompanied by the first sentence of Article 1 (4), the second sentence of Article 1 (5), the first sentence of Article 1 (6) and Article 1 (7). Article 1 (4), Article 1 (5), first sentence, Article 1 (6), second sentence and Article 1 (7) shall apply to the forms referred to in paragraph 2.
Efficacy
This Decree shall take effect on the date of its publication, with the exception of the provisions of Paragraph 4, which shall take effect on 1 September 2005.
