DECLARATION
of 4 October 2018
on training in the basic strains of doctors
According to § 37 (1) (n), (p) and (q) of Act No. 95 / 2004 Coll., on the conditions for obtaining and recognising professional competence and specialised competence for the medical profession of physician, dental practitioner and pharmacist, as amended by Act No. 67 / 2017 Coll.:
Requirements
(a) the breakdown and scope of training in the basic strains of doctors (hereinafter referred to as the "basic strains");
(b) the content of education in the core tribes;
(c) technical and material equipment and personnel security, which are a condition for obtaining accreditation for the implementation of specialisation training in basic strains;
(d) the type of workplace on which compulsory professional experience takes place in basic tribal education;
(e) the length of compulsory professional experience within the core strains;
(f) theoretical knowledge and practical skills which are subject to testing after completion of primary school education;
(g) the scope and content of the study obligations required to be completed before applying for the test after the completion of the basic tribe education;
are set out in the Annex to this Order.
Transitional provisions
(1) The completion of professional experience by doctors in the field of specialisation training after 30 June 2017 and at the latest on the date before the date of entry into force of this Decree shall be deemed to have completed compulsory professional experience under the training programmes in force before the date of entry into force of this Order by 30 April 2019.
(2) The completion of professional experience by doctors in the field of specialisation in paediatric education after 30 June 2017, and at the latest on the date before the entry into force of this decree, has been completed by 30 April 2019 according to the educational programmes of paediatric medicine in force before the date of entry into force of this decree, is considered to have completed compulsory professional experience in the field of pediatric medicine under this decree.
Efficacy
This decree shall take effect on the day of its publication.

The scope and content of the study obligations to be completed before applying for the examination after completion of the training in the basic strain of anaesthesiologic
1. Adoption of compulsory professional experience in the field confirmed by the trainer with the relevant specialised competence or trainer according to the provisions of § 38a (2) (c) of Act No. 95 / 2004 Coll., on the conditions for obtaining and recognising professional competence and specialised competence to pursue the medical profession of doctor, dental practitioner and pharmacist, as amended, in an accredited establishment within the scope of the training programme of 6 + 24 months or 6 + 18 + 16.
2. The completion of compulsory training activities referred to in Part I. (b).

Requirements for technical and material equipment and personnel security for workplaces which are a condition for obtaining accreditation to carry out specialist medical training
Workplace for compulsory professional experience in the field I (a) 1. - 4.
For Part I. (a) 1.
Anesthesiology and Intensive Medicine
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
(c) type of workplace
The health service provider provides medical services in the form of acute bed care intensive, intensive care 3rd degree -resuscitation care.
The health service provider shall provide anaesthesiological care in at least the field of surgery and in at least one of the following fields:
1. orthopaedic surgery and trauma of the musculoskeletal system,
2. otorinolaryngology and head and neck surgery,
3rd urology.
Health care in at least two of the above fields is provided in the form of bed care, this condition will also be fulfilled by workplaces with a common bed fund for these expertise.
The health service provider provides acute bed intensive and resuscitation care on beds of a multidisciplinary nature or a multidisciplinary intensive care unit with the possibility of ventilation, where the management is attended by a doctor anaesthesiologist with specialised competence in the field of anesthesiology and intensive medicine.
Acute intake to intensive / resuscitation beds: at least 100 patients per calendar year or on average 100 patients per year over the last 3 calendar years.
Anesthesia administered: at least 1 500 per calendar year or on average 1 500 per year for the last 3 calendar years.

Surgery
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
Health care shall be provided under a continuous working regime of at least:
1. a physician with specialised competence in surgery;
2. a doctor with a specialised competence in surgery or a doctor assigned to specialist training in surgery with at least 3 months' experience since being in the field,
3. a doctor with specialist competence in anesthesiology and intensive medicine,
4. a physician with specialised competence in radiology and imaging methods,
5. a doctor with specialised competence in the field of internal medicine,
6. a doctor with specialist competence in gyno and obstetrics.
(c) type of workplace
The health service provider provides emergency bed care in the field of surgery.
The facility has at least 2 operating rooms.
Components of the health service provider for which professional experience is carried out:
1. bed separation of anesthesiology and intensive medicine, or intensive care unit, or multidisciplinary unit, with possibility of at least short-term artificial pulmonary ventilation,
2nd Radiodiagnostic Division,
3. internal department,
4. endoscopy workplace,
5. Gynecology,
6. has services in the field: hematology and transfusion medicine, clinical biochemistry, medical microbiology and pathology.
Number of hospitalised patients per calendar year: at least 1 500 or an average of 1 500 per year for the last 3 calendar years.
Number of outpatient outpatient patients per calendar year: at least 15 000 or on average 15 000 per year for the last 3 calendar years.
List of performance types and their number per calendar year:
1. planned operations: 700 or on average 700 per year for the last 3 calendar years;
2. urgent operations: at least 200 or on average 200 per year for the last 3 calendar years,
of which laparoscopic operations urgent or planned: at least 100 or 100 per year for the last 3 calendar years,
3. trauma treatment: at least 500 of which at least 30 osteosynthesis or on average 500 per year for the last 3 calendar years.
and at the same time for Part I. (a) 3.
Internal medicine
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
At least 2 doctors who meet the conditions for internal medical trainers with a total minimum time of 1.5 for a given health service provider.
(c) type of workplace
The health service provider provides health care services in the field of internal medicine.
The health service provider shall meet the following care requirements:
1. income internal ambulance,
2. acute bed care with the condition of providing acute bed care intensive,
3. provision of basic bed care for patients in the field of internal medicine,
4. outpatient care in the field of internal medicine,
5. a diagnostic complement with 24-hour availability,
6. patient intake in the whole spectrum of internal diseases: at least 1000 income per health service provider per year or on average 1000 income per year over the last 3 calendar years,
7. outpatient care: at least 2000 patients per calendar year in ambulances providing professional care in the spectrum of internal diseases or on average 2000 patients per year for the last 3 calendar years.
and at the same time for Part I. (a) 4.
Anesthesiology and Intensive Medicine - 24 months
(a) technical equipment
The health service provider complies with the requirements for technical and physical equipment of the health care facility of the relevant field according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider complies with the requirements for minimum staff security of the health care facility of the relevant field according to Decree No. 99 / 2012 Coll.
(c) type of workplace
The health service provider provides medical services in the form of emergency bed care intensive, intensive care in the 3rd degree - resuscitation care.
The health service provider shall provide anesthesiological care for at least 4 of the following fields where the health service provider provides bed care:
1. surgery,
2. gynecology and obstetrics,
3. orthopaedic surgery and trauma of the musculoskeletal system,
4. otorinolaryngology and head and neck surgery,
5th urology.
The workplace provides acute bed care intensive in beds of a multidisciplinary character, or in a multidisciplinary intensive care unit under the guidance of a doctor anaesthesiologist.
Acute intake to intensive / resuscitation beds: at least 100 patients per calendar year or on average 100 per year for the last 3 calendar years.
Anesthesia administered: at least 4 000 per calendar year or on average 4000 per year for the last 3 calendar years.
and, where appropriate, for Part I (a) (4).
Urgent medicine
(a) technical equipment
The health service provider meets the requirements for technical and material equipment of urgent income according to Decree No. 92 / 2012 Coll.
(b) Personnel security
The health service provider meets the requirements for minimum personnel security of urgent income according to Decree No. 99 / 2012 Coll.
Educational activities with entrusted organisations
Medical first aid course
(a) technical equipment
Textroom for theoretical instruction.
A practical training room with equipment: a model (adult, infant and newborn) enabling practical training of basic and extended emergency resuscitation with simultaneous recording of monitored vital functions, in particular respiratory and circulatory to objectify the effectiveness of resuscitation and storage of monitored data into the PC and final evaluation.
The model shall allow for training:
1. ensuring airway passage through air ducts, Combi- tubus, laryngeal tube, laryngeal mask, including intubation and various tracheal intubation techniques,
2. artificial pulmonary ventilation from the lungs to the lungs through the mouth, through the mask, by hand breathing apparatus / transport fan,
3. child intubation practice including neonates and artificial pulmonary ventilation,
4. ensuring airway through the airways of coniopunctions, minitracheotomy (cricket),
5. pneumothorax puncture,
