on the model of the card of the staff of the Czech Telecommunications Office responsible for electronic communications and postal services control
The Ministry of Industry and Trade provides pursuant to Section 150 (2) of Act No. 127 / 2005 Coll., on Electronic Communications and on the amendment of certain related laws (Act on Electronic Communications), as amended by Act No. 153 / 2010 Coll., Act No. 468 / 2011 Coll., Act No. 214 / 2013 Coll. and Act No. 258 / 2014 Coll., as amended by Act No. 258 / 2014 Coll., as amended by Act No. 113 (8) on Electronic Communications, and pursuant to Section 41 (3) of Act No. 29 / 2000 Coll., on Postal Services and amending certain laws (Act on Postal Services), as amended by Act No. 258 / 2014 Coll., as amended by Act No. 37 (2):
The model of the licence of the staff of the Czech Telecommunications Office responsible for the control of electronic communications and postal services is set out in the annex to this decree.
They shall be deleted:
1. Decree No. 159 / 2005 Coll., laying down the model of the certificate for the performance of state control of electronic communications.
2. Decree No. 26 / 2006 Coll., amending Decree No. 159 / 2005 Coll., laying down the model of the licence to carry out state control of electronic communications.
This Regulation shall enter into force on 1 September 2017.

Model of licence of the staff of the Czech Telecommunications Office responsible for electronic communications and postal services control
1. Description
a) On the face side of the licence there is a photograph of the Czech Telecommunications Office employee in the upper left. The requirements for the technical execution of the photograph shall comply with the technical requirements for photographs for the issue of the identity card. In the central part of the licence, the name of the inspection authority shall appear on the first line as "THE CZECH TELECOMMUNICATION AUTHORITY '. The second line on the right shows the registration number of the licence. The third line shall bear the identification of the licence by the words" EMPLOYER TRADE'. The fourth line shows the name, surname and title of the employee. The fifth line on the left shows "Other records" and the employee's function in the middle. A large national emblem of the Czech Republic is located in the right part of the licence at the level of the fourth and fifth lines. The lower part of the licence contains the words "Send the document found to the Czech Telecommunications Office, based in Sokolovská 219, Prague 9, postal box 02, 225 02 Praha 025."
(b) On the reverse side of the licence, the words "EMPLOYMENT CERTIFICATE" shall appear on the first to third lines, the words "EMPLOYMENT CERTIFICATE" and on the fifth row of the word "Ev. No." including the relevant registration number of the licence. On the sixth line, the text "is authorised by the person responsible for carrying out control of electronic communications and postal services." In the lower part, the place and date of issue are indicated on the left by the words "In Prague on" and on the right by the function and signature of the President of the Council of the Czech Telecommunications Office.
(c) The card of the employee responsible for the electronic communications and postal service inspection shall be printed on a white plastic adhesive film placed on a rectangular card with rounded corners of 85,6 mm x 54 mm. The printing of the face and reverse pages is made on a white background, on the back of the licence the logo of the Czech Telecommunications Office is placed in the background of the texts in colour and the label "Czech Telecommunications Office" is placed at the bottom.
