DECLARATION
of 9 June 2010
amending Decree No. 223 / 2005 Coll., on certain educational documents, as amended
The Ministry of Education, Youth and Sports provides, pursuant to § 28 (7) of Act No. 561 / 2004 Coll., on pre-school, primary, secondary, higher vocational and other education (Education Act), as amended by Act No. 179 / 2006 Coll.:
Amendment of the Decree on certain educational documents
Decree No. 223 / 2005 Coll., on certain educational documents, as amended by Decree No. 489 / 2006 Coll. and Decree No. 63 / 2008 Coll., is amended as follows:
1. in Paragraph 1 (3) (c):
"(c) the certificate of discharge and the diploma of discharge in the Conservatory, the models of the certificate of graduate examination of pupils who were due to complete secondary education through the graduate examination before 1 September 2010, are set out in Annex 8 to this Decree, '.
2. In Article 1 (3), the dot is replaced by a comma at the end of point (d) and the following point (e) is added:
"(e) The Certificate of Graduate Trial for Students who have duly completed secondary education by means of a Master Trial after 1 September 2010 are set out in Annex No 8a to this Order."
3. Paragraph 3 (4), including footnotes 5 and 7, reads:
"(4) Where a school which has issued an original is no longer in existence, a copy shall be issued in accordance with the procedure laid down in paragraphs 2 and 3 by the school which imposes its documentation, or by another school providing education in the same or similar field of education, provided that the applicant submits to him copies of the documents on the basis of which the copy may be issued, drawn up in accordance with the Charter and File Service Act and amending certain Laws (5) or certified under the Verification Act (7).

7) Act No. 21 / 2006 Coll., on the verification of the conformity of a copy or copy with the instrument and on the verification of the authenticity of the signature and on the amendment of certain laws (the Verification Act), as amended. '
4. Paragraph 3 (6), including footnotes 5a and 5b, reads as follows:
"(6) Where a copy is issued because of a change in the name, surname or birth number, the applicant's name and surname and the birth number shall be indicated on the copy as at the date of issue of the copy. The application for the issue of a copy shall be accompanied by the original of the certificate, the certificate or the diploma of discharge, and by evidence of the change of name, surname and birth number. The change of name or surname shall be evidenced by a decision of the competent administrative authority authorising the change of name and surname (5a) or, where appropriate, an officially certified copy thereof. A change in the birth number shall be evidenced in accordance with other legislation5b).

5. In Annexes 1 to 3, in the model forms of the certificate in the Polish language, the word "Sprawowanie 'is replaced by" Zachowanie'.
6. In Annex 1, in the models of forms Certificate list A 1st degree subjects - Polish and Certificate list A 1st degree - Polish, in Annex 2, in the model forms A 2nd degree subjects RVJ - Polish, Certificate list A 2nd degree - Polish and Certificate list A 2. degree objects - Polish with the words "Uwaga: Świadectwo - arkusz A jest ważne samodzielnie, pod warunkiem, że ocenę wyników kształcenia ucznia wyrażono tylko stopniami ze skali ocen. Ocenę slovną wyników kształcenia ucznia w jednym lub z kilku przedmiotach obowiązkowych, nadobowiązkowych i kółek zainteresowań zaznacza się w Świadectwie - arkusz B, który w tym przypadku jest nieoddzielna częścią składową świadectwa - arkusz A." replaced by "Uwagi: Świadectwo - arkusz A - jest ważne samodzielnie, pod warunkiem, że ocenę wyników kształcenia ucznia wyrażono tylko stopniami ze skali ocen. Ocenę opisową wyników kształcenia ucznia z jednego lub z kilku przedmiotów obowiązkowych, nadobowiązkowych i kółek zainteresowań zaznacza się w Świadectwie - arkusz B, który w tym przypadku jest nieoddzielną częścią składową świadectwa - arkusz A. '
7. In Annexes 1 and 2, in the model forms of the certificate in Polish, the words "Opuszczone lekcje 'are replaced by the words" Opuszczone godziny lekcyjne'.
8. In Annexes 1 and 2, in the model forms of the certificate in the Polish language, the words "nie usprawiedliwione 'are replaced by the words" nieusprawiedliwione'.
9. In Annexes 1 and 2, in the model forms of the certificate in Polish, a comma is inserted after the words "Przedmioty nadobowiązkowe '.
10. In Annexes No 1 and 2, in the model forms, the words "Ocena wyników nauczania wyrażonych stopniami i opisowo 'are replaced by the words" Ocena wyników nauczania wyrażona stopniami klasyfikacji oraz oceną opisową'.
11. In Annexes 1 and 2, in the models of the forms of the certificate in Polish, the words "SKALA OCEN ZACHOWANIA I POSTZEDMIOTACH ZKOWYCH I NADOBOBOWIC ZKOWYCH 'shall be replaced by the words" SKALA OCEN Z ZACHOWANIA I FROM POSTZEANIA I (W PRZEDMIOTACH OBOWIC ZKOWYCH I NADOBOBOWIC ZKOWYCH)'.
12. In Annexes Nos 1 and 2, in the model forms, the words "otrzymał (a) promoję z odznaczeniem 'are replaced by the words" otrzymał (a) promokję z wyróżnieniem'.
13. In Annex 3, in the model forms of the certificate in Polish, the words "z tego nie usprawiedliwiono 'are replaced by the words" z tego nieusprawiedliwionych'.
14. In Annex 3, in the model forms, the words "Ocena wyników nauczania wyražzonych stopniami 'are replaced by" Ocena wyników nauczania wyrażona opisowo'.
15. In Annex 3, in the model forms of the certificate in Polish, the words "Liczba opuszczonych lekcji 'are replaced by the words" Liczba opuszczonych godzin lekcijnych'.
16. In Annex 3, in the model forms of the certificate in Polish, the words "z tego nie usprawiedliwiono 'are replaced by the words" z tego nieusprawiedliwionych'.
17. In Annex 3, the words "Uznane wyksztalcenie osiagniete poprzednio 'are replaced by the words" Udowodnione i uznane wykształcenie osiągnięte wcześniej'.
18. In Annex 3, in the models of the forms of the certificate in Polish, the words "Przynadnost panstwowa 'are replaced by the words" Obstatelstwo'.
19. In Annex 7, in the model form of the graduate diploma, the words "(in short," DiS ") 'are replaced by" DiS.';
20. In Annex 8, the words "Model forms referred to in § 1 (3) (c) and § 5 (2) (c) of the Certificate of discharge, the diploma of discharge in the Conservatory and the certificate of graduation 'are replaced by the words" Model forms referred to in § 1 (3) (c) of the Certificate of discharge, the diploma of discharge in the Conservatory and the certificate of graduate examination of pupils who should have duly completed secondary education through the examination before 1 September 2010'.
21. The following Annex 8a is inserted after Annex 8:

Efficacy
This Decree shall take effect on 1 July 2010.
