DECLARATION
of 7 June 2012
amending Decree No. 424 / 2011 Coll., on the template, formalities and execution of the social system card, the template, the particulars and the completion of the confirmation of the loss, theft, damage or destruction of the social systems card and the model of the Standardised record of the social worker
The Ministry of Labour and Social Affairs provides pursuant to § 4b (7) of Act No. 73 / 2011 Coll., on the Labour Office of the Czech Republic and on the amendment of related laws, as amended by Act No. 366 / 2011 Coll.:
Decree No. 424 / 2011 Coll., on the template, requirements and execution of the Social Systems Card, the template, the elements and the execution of the certificate of loss, theft, damage or destruction of the Social Systems Card and the model of the Standardised Record of Social Workers, is amended as follows:
1. In Paragraph 1, the current text becomes paragraph 1 and the following paragraph 2 is added:
"(2) Instead of a plastic card, the card referred to in paragraph 1 (b) shall be issued as a paper card for the period up to 31 December 2012, which is sealed in a transparent laminating sheet. ';
2. In Paragraph 2 (2), the words "19,1 mm x 19,1 mm 'are replaced by the words" 23 mm x 17,8 mm' and at the end of paragraph 2, the words "The card referred to in Paragraph 1 (2) shall also bear an official stamp on the front and the signature of the authorised official. ';
3. in § 2 (4), § 4 and § 5, the words "under § 1 (a)" shall be replaced by the words "under § 1 (1) (a)";
4. In Article 4, "Article 1 (b) 'is replaced by" Article 1 (1) (b)';
5. In Article 5, "Article 1 (b) 'is replaced by" Article 1 (1) (b) and (2)';
6. Annexes 1 and 2 shall read:

Transitional provision
Paper cards with the function of the card of a disabled person, issued from 1 January 2012 until the date of entry into force of this decree, shall be considered as paper cards pursuant to Article 1 (2) of Decree No. 424 / 2011 Coll., as effective from the date of entry into force of this decree.
This decree shall take effect on the day of its publication.
