DECLARATION

on the content elements of the application for authorisation for domestic slaughter of bovine animals under 24 months of age or of deer from farmed farming and the content requirements for its notification
The Ministry of Agriculture provides pursuant to Article 78 of Act No. 166 / 1999 Coll., on Veterinary Care and on the amendment of certain related laws (Veterinary Act), as amended by Act No. 131 / 2003 Coll., Act No. 316 / 2004 Coll., Act No. 48 / 2006 Coll., Act No. 182 / 2008 Coll., Act No. 227 / 2009 Coll., Act No. 308 / 2011 Coll. and Act No. 359 / 2012 Coll., ("the Act ') for the implementation of Section 21 paragraph 3 of the Act:
Subject matter
This decree regulates the content of the breeder's application for authorisation to carry out domestic slaughter of bovine animals under 24 months of age or of deer from farmed farming (hereinafter referred to as "domestic slaughter") and the content of the reporting of domestic slaughter.
Content of the application
The application for authorisation to carry out domestic slaughtering shall contain, in addition to the formalities provided for in the Administrative Rules (1).
(a) telephone or e-mail links to breeders,
(b) the address of the holding and, where appropriate, the other destination of the holding, if the address cannot be indicated,
(c) the holding registration number.
Content details of the notification
Notification of domestic slaughter shall include, in addition to the particulars referred to in Section 2:
(a) date and time of domestic slaughter;
(b) the number, type and identification number of the animal slaughtered and, in the case of bovine animals, the date of birth;
(c) the method of disposal of specified risk material2) if it is about the slaughter of bovine animals.
Efficacy
This decree shall take effect on the day of its publication.
