According to Section 374a (e) of Act No. 99 / 1963 Coll., the Civil Code, as amended by Act No. 123 / 2008 Coll., ("the Act '), provides for:
The model of the application form for the issue of an electronic payment order for the procedure provided for in Article 174a (1) of the Act is set out in the annex to this order and published on the website of the Ministry of Justice "http: / / www.justice.cz."
This Decree shall take effect on 1 July 2008.
