laying down the model of the certificate of the insolvency administrator
The Ministry of Justice provides pursuant to Article 6 (8) of Act No. 312 / 2006 Coll., on Insolvency Managers, as amended by Act No. 31 / 2019 Coll.:
Model of the Insolvency Trustee Card
(1) The model of the Insolvency Trustee licence issued to a natural person who is the Insolvency Trustee is set out in Annex 1 to this Order.
(2) The model of the Insolvency Trustee Card issued to a natural person who is a notified member of the Insolvency Trustee is set out in Annex 2 to this Order.
Efficacy
This decree shall take effect on the 15th day following its publication.

Model of the Insolvency Trustee Card issued to a natural person who is an Insolvency Trustee
1. Description
(a) The Insolvency Administrator card is a two-sided plastic card of light blue colour 85.6 x 54 mm with rounded corners. The Insolvency Administrator card is secured by a protective element with the motif of a silver crowned two-tail lion in the jump and the text "HAS DELIVERED THE MINISTERST OF THE JUSTICE OF THE CZECH REPUBLIC '.
(b) The words "INSOLVENTION MANAGER" shall appear on the face of the insolvency administrator's licence on the first to second lines. The third line contains the text "RECEIVATION 'and the text" IČO'. The fourth line contains the text "NAME 'and the text" TRADE NUMBER'. The fifth line contains the text "TINUL 'and the text" VALIDITY DO'. In the left part there is digital processing of colour photography of the insolvency administrator with dimensions 20 × 25,8 mm. The requirements for the technical execution of the photograph are proportionate to § 7 (2), (3) and (4) of Decree No. 400 / 2011 Coll., implementing the Civil ID Act and the Travel Document Act. Under the fourth line on the left, the name or names shall be entered. The figures below the fifth line on the left shall be entered in the format of the academic title given before the name, the academic title given after the name. The validity of the licence shall be the same as that of the permit or special authorisation of the natural person issuing the licence. Date of validity shall be indicated in the format dd.mm.yyyy. All texts, including written data, shall be drawn up in black and blue characters, except for the text "INSOLVENTION MANAGEMENT ', which shall be drawn up in pasted characters. In the background of the RS is shown the theme of the lime leaf.
c) In the background of the reverse side is shown the theme of the curve and the symbol of the paragraph.

Model of the Insolvency Trustee card issued to a natural person who is a notified member of the Insolvency Trustee
1. Description
a) The Insolvency Administrator card is a double-printed plastic card of dark blue colour 85.6 x 54 mm with rounded corners. The Insolvency Administrator card is secured by a protective element with the motif of a silver crowned two-tail lion in the jump and the text "HAS DELIVERED THE MINISTERST OF THE JUSTICE OF THE CZECH REPUBLIC '.
(b) The first line of the "INSOLVENTION GOVERNANCE MANAGER" licence shall appear on the face of the reported member of the insolvency administrator. The second line contains the text "NOTIFIED COMPANY OF THE INSOLVENCY GOVERNMENT '. The third line shows the firm of the insolvency administrator. The fourth line shows the text" RECEIVATION' and the text "IČO '. The fifth line contains the text" NAME' and the text "TRADE NUMBER '. The sixth row contains the text" TITULE' and the text "VALIDITY DO '. In the left-hand side there is a digital processing of a colour photograph of the reported member of the insolvency administrator to whom the licence is issued, measuring 20 × 25,8 mm. The requirements for the technical execution of the photograph are proportionate to § 7 (2), (3) and (4) of Decree No. 400 / 2011 Coll., implementing the Civil ID Act and the Travel Document Act. Under the fourth line on the right, the identification number of the insolvency practitioner, the natural person to whom the licence is issued, shall be indicated by the reported partner. Under the fifth line on the left, the name or names are entered. The data below the sixth line on the left shall be entered in the format of the academic title before the name, the academic title after the name. The validity of the licence shall be the same as that of the permit or special authorisation of the natural person issuing the licence. Date of validity shall be indicated in the format dd.mm.yyyy.
