DECLARATION
of 21 June 2017
amending Decree No 9 / 2015 Coll., laying down the formalities and models for the mandates for the exercise of state supervision, state professional supervision, top state supervision and top state professional supervision in transport in the form of a licence and amending the related decrees, as amended by Decree No 80 / 2017 Coll.
The Ministry of Transport provides, pursuant to Section 91 (1) of Act No. 56 / 2001 Coll., on the conditions for the operation of vehicles on the road and on the amendment of Act No. 168 / 1999 Coll., on the liability insurance for damage caused by the operation of the vehicle and on the amendment of certain related laws (Act No. 170 / 2007 Coll., Act No. 297 / 1999 Coll., Act No. 103 / 2004 Coll., Act No. 411 / 2005 Coll., Act No. 226 / 2006 Coll., Act No. 170 / 2007 Coll., Act No. 297 / 2009 Coll., Act No. 152 / 2011 Coll., Act No. 239 / 2013 Coll., and Act No. 63 / 2017 Coll., ("Act ') for the implementation of Section 82 (1):
Decree No 9 / 2015 Coll., on the establishment of the formalities and models of mandates for the exercise of state supervision, state professional supervision, top state supervision and top state professional supervision in transport in the form of a licence and amending the related decrees, as amended by Decree No 80 / 2017 Coll., shall be amended as follows:
1. The following Section 8a is inserted after Section 8:
Models of mandate for the performance of state professional supervision and top state supervision on road traffic conditions in the form of a licence
(K § 82 (1) of Act No. 56 / 2001 Coll.)
(1) The authorisation for the performance of state professional supervision and overseeing of road traffic conditions in the form of a licence shall include:
(a) an indication of the administrative authority which issued the mandate, over which the text "CZECH REPUBLIC" is given,
(b) an indication that it is a mandate for the exercise of national professional supervision or national supervision in respect of road traffic conditions;
(c) the authorisation number,
(d) the name and surname of the delegate;
(e) a photograph of the authorised person measuring 35 x 45 mm, the lower edge of which is glued with a holographic sticker with the text "CZECH REPUBLIC,"
(f) the date of issue of the mandate,
(g) information on the authorisation of the delegate;
(h) the signature of the person empowered to issue the mandate;
(i) the print of the official stamp of the administrative authority which issued the mandate.
(2) The authorisation for the performance of state professional supervision and top-level surveillance on road traffic conditions in the form of a licence is a double-sided paper card with dimensions of 70 x 100 mm, which is sealed in a transparent laminating sheet of 75 x 105 mm. The card is equipped with protective features, which are embossed underprint, guilloche, micropísmo, in the UV spectrum of the illuminating colour, price paper with watermark positives x negative, containing two types of invisible fibres in the UV spectrum, transparent mark and numbering in the black light green in the UV spectrum.
(3) The models of the mandate for the exercise of state professional supervision and overseeing of road traffic conditions in the form of a licence are set out in Annex 9 to this Regulation. "

Efficacy
This Regulation shall enter into force on 1 July 2017.
