amending Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related acts, as amended
According to § 96 (f), (i) and (l) of Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the amendment of certain related laws, as amended by Act No. 312 / 2013 Coll.:
Decree No. 207 / 2001 Coll., implementing Act No. 301 / 2000 Coll., on Matrices, Name and Surname and on the Amendment of Certain Related Acts, as amended by Decree No. 230 / 2001 Coll., Decree No. 499 / 2001 Coll., Decree No. 566 / 2002 Coll., Decree No. 660 / 2004 Coll., Decree No. 539 / 2005 Coll., Decree No. 300 / 2006 Coll., Decree No. 352 / 2006 Coll., Decree No. 389 / 2007 Coll., Decree No. 455 / 2008 Coll., Decree No. 445 / 2008 Coll., Decree No. 485 / 2009 Coll., Decree No. 315 / 2010 Coll., Decree No. 417 / 2011 Coll., Decree No. 455 / 2012 Coll., Decree No. 424 / 2013 Coll., amended as follows:
1. The following Section 2a is inserted after Section 2, including the title:
Forms
(1) The matrix forms referred to in Annex 2 in models 9 to 14 and 20 are strictly negotiable forms with security features against counterfeiting and alteration. Each form shall bear a letter or letters identifying the series and a six-digit number of each series from 000001 to 999999. After the numbers of the series are exhausted, the series marked in alphabetical order.
(2) The matrix forms referred to in paragraph 1 are printed on 100% pulp A4-free paper, 110 g / m2, without optical brightener with full chemical protection.
(3) In addition to the full chemical protection of the paper, the security features of the matrix forms referred to in paragraph 1 shall be the unique and unmistakable continuous watermark of the lion in the shield and of the leaves.
(4) The Registry shall keep an overview of the forms received, issued, damaged or degraded as referred to in paragraph 1; it shall destroy the damaged and degraded forms in a protocol. "
2. In Section 10, the word "surname 'is deleted.
3. Paragraph 10 (4) is deleted.
Paragraph 5 shall become paragraph 4.
4. Paragraph 19 (2) is deleted.
Paragraph 3 shall become paragraph 2.
5. In Annex 2 to Model 7, the word "dead 'is replaced by the word" deceased'.
6. In Annex No 2, models 9 to 14 read:

7. In Annex 2, Model 16, point 6:
'6. Proof of legal capacity to marry, if any; Certificate of legal capacity for marriage, if it is a citizen of the Czech Republic when marriage is concluded at the representative office of the Czech Republic abroad.
6. Evidence of legal capacity to marry, if any; Certificate of legal capacity for marriage, if it is a Czech citizen when marriage is concluded at the Czech representative's office abroad. "
8. In Annex No 2, Model 20 reads:

Transitional provision
Matrix forms of the Death Matrix List issued under existing legislation may be used until 31 December 2035 at the latest, provided that the amendments referred to in Article I (5) are indicated.
Efficacy
This Decree shall enter into force on 1 January 2015.
