implementing the Act on the Registration of Citizens, the Act on Civil Documents and the Act on Travel Documents
According to Section 25 (a) of Act No. 133 / 2000 Coll., on the registration of residents and birth numbers and on the amendment of certain laws (Act on the registration of residents), § 5 (1) (b), § 12 (3) and § 26 of Act No. 328 / 1999 Coll., on Civil Documents, and § 3 (4), § 5 (3) and § 20 (4) of Act No. 329 / 1999 Coll., on Travel Documents and on the amendment of Act No. 283 / 1991 Coll., on the Police of the Czech Republic, as amended, (Act on Travel Documents):
Model civil ID
Model of identity card with machine-readable data
(1) The model of the identity card with machine-readable data is set out in Annex 4.
(2) The machine-readable data are recorded at the bottom of the ID card.
(3) The data referred to in § 3 (3) and (4) of Act No 328 / 1999 Coll., on civil ID, are entered on the back of the identity card.
Model ID card without machine-readable data
The model of the identity card without machine-readable data is set out in Annex 5.
(1) The marked part of the identity card which is separated when the change of residence is declared shall be understood as the left upper corner of the back of the identity card referred to in Paragraph 2 (1), marked A.
(2) The marked part of the identity card which is separated when the family is changed by marriage or, where appropriate, by the change of surname in connection with the change of marriage, means the upper right-hand corner of the back of the identity card referred to in Section 2 (1), marked "B."
Model of the certificate and model of the application
(1) The model of the certificate of identity card is set out in Annex 6.
(2) The model of the certificate of change of family status and, where appropriate, the change of surname in connection with the change of marriage is set out in Annex 7.
(3) The model of the application for the issue of a card is set out in Annex 8.
Requirements for the technical execution of the photograph for the issue of the identity card
(1) The photograph for the issue of the identity card has a smooth surface and is in black and white or colour. The photograph is rectangular with dimensions 35 mm x 45 mm, built on a shorter side with straight or rounded corners with R = 3 mm ± 0,5 mm with thickness from 0,13 mm to 0,27 mm. The image dimensions are shown in Annex 9, Figure 1.
(2) The photograph shows the head and shoulders of the person displayed. The person is shown in the front view in such a way that the central vertical plane of the face is an extension of the vertical plane of the lens of the sensing system and the horizontal plane of the face formed by the eyes is an extension of the horizontal plane of the lens of the sensing system. The position of the lens of the sensing system vis-à-vis the person is shown in Annex 9, Figure 2. The view of the person displayed is directed to the lens.
(3) The distance between the upper edge of the head and the upper edge of the photograph is at least 2 mm. The height of the facial part of the head, which consists of a distance from eyes to chin, shall be at least 13 mm. The size of the person's head, including its location, is given in Annex 9, Figure 3.
(4) The background behind the displayed person is white to light blue, allowing for smooth transition of these colors. The photo is without glare which would hide or significantly change the characteristic identification characteristics of a person, which are in particular the shape and location of eyebrows, eyes, nose, mouth, chin and the overall shape of the face. Could not perform retouches and other modifications of the negative and positive photo.
Model travel documents
Passport with machine-readable zone
(1) The model of the machine-readable passport is set out in Annex No 11.
(2) The machine-readable data are recorded at the bottom of the data page, which is located on the other side of the passport plates.
(3) On page 31 of the passport, the title of the graduate of the higher vocational school, the academic degree, the status mark, another degree of the university graduate or the scientific rank shall be registered.
Passport without machine-readable zone
The model of a passport without a machine-readable zone is set out in Annex 12.
(1) The passports referred to in Sections 8 and 9 are travel documents of the type of book and contain 32 pages.
(2) Children are registered on page 31 of the passports referred to in Sections 8 and 9.
Travel card and other travel document under an international contract
(1) The model of the travel document is set out in Annex 13.
(2) The travel document is a travel document of a type of book and contains 16 pages.
(3) The model of another travel document issued under an international contract, unless provided for in this Treaty, is set out in Annex 14.
Models of application forms
(1) The model of the passport application form is set out in Annex 15.
(2) The model of the application form for a travel document is set out in Annex No 16.
(3) The model forms for applications for the issue of other travel documents issued under international agreements are set out in Annex 17.
(4) The model of the application form for changes to the travel document is set out in Annex 18.
Requirements for the technical execution of the photograph attached to the travel document application
The requirements for the technical execution of the photograph attached to the travel document application are subject to the provisions of Section 6.
Repeal
They shall be deleted:
1. Decree No. 146 / 1982 Coll., implementing the Act on the reporting and registration of residents.
2. Decree No. 128 / 1993 Coll., implementing the Civil ID Act.
3. Decree No. 129 / 1993 Coll., supplementing Decree No. 146 / 1982 Coll., implementing the Act on the reporting and registration of residents.
4. Decree No. 213 / 1994 Coll., amending Decree No. 128 / 1993 Coll., implementing the Civil ID Act.
5. Decree No. 50 / 1995 Coll., amending Decree No. 146 / 1982 Coll., implementing the Act on the reporting and registration of residents, as amended by Decree No. 129 / 1993 Coll.
6. Decree No. 174 / 1999 Coll., amending Decree No. 128 / 1993 Coll., implementing the Act on Civil Documents, as amended by Decree No. 213 / 1994 Coll.
Efficacy
This Decree shall take effect on 1 July 2000.
