DECLARATION
of 12 May 2008
amending Decree No 495 / 2006 Coll., laying down models for diplomatic and service passports and forms for applications for extradition
The Ministry of Foreign Affairs provides pursuant to § 5 (6) (b) and § 20 (8) of Act No. 329 / 1999 Coll., on Travel Documents and amending Act No. 283 / 1991 Coll., on Police of the Czech Republic, as amended, (Act on Travel Documents), as amended by Act No. 136 / 2006 Coll. and Act No. 106 / 2007 Coll.:
Decree No 495 / 2006 Coll., on the establishment of models of diplomatic and service passports and forms of applications for extradition, is hereby amended as follows:
1. In Article 2, the following paragraph 5 is added:
"(5) The model of the application form for changes to the data and entry of the child in the diplomatic or service passport is set out in Annex 9 to this Order. '
2. Annex 7 shall read as follows:
"Annex No 7 to Decree No 495 / 2006 Coll.
3. Annex 8 shall read as follows:
"Annex No 8 to Decree No 495 / 2006 Coll.
4. The following Annex 9 is added:
"Annex No 9 to Decree No 495 / 2006 Coll.
This decree shall take effect on the day of its publication.
