DECLARATION
of 19 April 2005
laying down the model of the licence for the exercise of State control of electronic communications
According to Section 150 (2) of Act No. 127 / 2005 Coll., on Electronic Communications and on the amendment of certain related laws (the Act on Electronic Communications) ("the Act '), the Ministry of Informatics provides for the implementation of Section 113 (9) of the Act:
(1) The document for the performance of the State control of electronic communications (hereinafter referred to as the "licence"), proving by authorised persons (1) in the performance of the State control of electronic communications, is printed on a white rectangular card of dimensions 82 mm x 51 mm. The card is covered with clear lamination foil with rounded corners.
(2) The printing of the front page is made on a white basis with a large national emblem of the Czech Republic and entitled "CZECH REPUBLIC - CZECH TELECOMMUNICATIONS AUTHORITY" at the top of the licence. In the middle of the front of the lower part of the licence is glued round transparent holographic film with 20 mm size, on which appears the trademark of the Czech Telecommunications Office. The left-hand side of the front shall include the text "Certificate for State Control of Electronic Communications', the surname and name of the authorised person, the licence number and the place and date of issue. There is a photograph of the authorised person on the front of the licence on the right; for the technical execution of the photograph, the provisions of the special legislation on the requirements for the technical execution of the photograph for the issue of the ID 2 shall apply mutatis mutandis).
(3) The back of the licence contains an instruction for mandatory persons (1), the signature of the President of the Council of the Czech Telecommunications Office, the signature of the authorised person and the instruction for the finder of the licence.
The model of the licence is set out in the Annex to this Order.
Efficacy
This Decree shall take effect on 1 May 2005.
